O processo de descentralização da gestão do Sistema Único de Saúde (SUS) tem como ferramenta para seu aprofundamento a regionalização em saúde. Embora proposta constitucionalmente em 1988 na criação do SUS, apenas nos últimos anos foi resgatada e fomentada através de documentos oficiais indutores da política de saúde. No entanto, o sistema nacional de saúde brasileiro se configurou como um mix público-privado e poucos são os estudos que procuram analisar esta realidade compreendendo a relação existente entre ambos e sugerindo mecanismos de fortalecimento do SUS diante da presença relevante do setor suplementar, principalmente no Estado de São Paulo. The use of wavelet analysis is very common in a large variety of disciplines, such as signal and image processing, quantum mechanics, geophysics, medicine, biology, etc. In economics, however, wavelets are still a mysterious, but colorful, tool for time-series analysis. The pioneering work of Ramsey and Lampart [26] is unknown to the majority of economists. Among the exceptions to this rule, one can point to [4], [14], and [12]. See [6], for a recent survey of wavelet applications to economic data. Probably, wavelets are not more popular among economists, because wavelet multivariate analysis is still incipient. Recently, however, Gallegati [11] — using the maximum overlap discrete wavelet transform — and Crowley and Mayes [5] and Aguiar-Conraria and Soares [1] — using the continuous wavelet transform — showed how the cross-wavelet analysis could be fruitfully used to uncover time-frequency interactions between two economic timeseries. Still, most surely, wavelets will not become very fashionable in economics until a concept analogous to the spectral partial-coherence is developed. Dado o crescente cenário competitivo no mundo empresarial, as organizações são obrigadas a implementar mudanças que lhes permitam manter-se no mercado. Tais mudanças exigem alterações comportamentais e de relacionamento, que requerem ajustamentos individuais que, geralmente, provocam insegurança e stress nos indivíduos abrangidos (Camara, Guerra, & Rodrigues, 2010). O presente estudo teve como objectivo geral aceder às representações que os trabalhadores do Centro Hospitalar do Nordeste [CHNE] têm da mudança organizacional, mormente, quanto ao impacto desta na saúde e bem-estar e também, compreender como os trabalhadores analisam a concepção, implementação e gestão da mudança organizacional. Para o efeito privilegiou-se a metodologia quantitativa, essencialmente descritiva, com a realização de um inquérito por questionário, utilizando alguns itens do instrumento de Torres-Oliveira (2005). Many library data processing systems are unresponsive to the needs of librarians because of the necessity to batch-process transactions in a computer center. Such systems tend to be reportoriented rather than information-oriented with resultant problems in the timeliness of information. Time-shared computing permits multiple users to process jobs simultaneously through on-line interaction with the computer. Such systems offer to the librarian the advantages of immediate access to information, costs shared with other users, and direct man-machine interaction. This tutorial paper describes time-shared systems with applications in the library. Problems concerning the cost and present state-of-the-art of time-sharing are discussed.

In an effort to combat the frustration experienced by educators and students alike with the volume of factual and practical information the student is expected to learn and apply, new medical teaching methods are being developed. Research in the field of adult education techniques has determined which teaching methods best maintain learner interest and result in the greatest knowledge retention. Electronic data processing technology allows for the rapid organization and specific acquisition of vast amounts of information. Between November 2005 and October 2006, a total of 106 academic and corporate researchers requested the services of the CRCHUL Genomic Sequencing and Genotyping Platform. All procedures are performed by only one manager and two research assistants. During this one-year period, we sequenced 205,000 DNA samples using only one ABI 3730xl DNA analyzer. We also genotyped 8200 samples using one ABI 3100 sequencer. The maximum delivery time was 48 h for all projects of less than 384 samples. These samples comprised 80% of all our orders. The management and integration of all these samples were conducted on a computer system that we developed in house. This laboratory information management system has significantly reduced processing and analysis time and concurrently increased production capacity. All data are stored on our Oracle 10G database installed on a UNIX server, which is connected to an interface application server module to increase security. The Web interfaces were developed in PL/SQL, which facilitates quick consultations, additions, and modifications. Our customers can thus easily submit their samples at www.sequences.crchul.ulaval.ca. All laboratory procedures are tracked and managed by two research assistants through our laboratory information management system (LIMS). Removal of the anterior temporal lobe (ATL) is an effective surgical treatment for intractable temporal lobe epilepsy but carries a risk of language and verbal memory deficits. Preoperative localization of functional zones in the ATL might help reduce these risks, yet fMRI protocols in current widespread use produce very little activation in this region. Based on recent evidence suggesting a role for the ATL in semantic integration, we designed an fMRI protocol comparing comprehension of brief narratives (Story task) with a semantically shallow control task involving serial arithmetic (Math task). The Story > Math contrast elicited strong activation throughout the ATL, lateral temporal lobe, and medial temporal lobe bilaterally in an initial cohort of 18 healthy participants. The task protocol was then implemented at 6 other imaging centers using identical methods. Data from a second cohort of participants scanned at these centers closely replicated the results from the initial cohort. The Story-Math protocol provides a reliable method for activation of surgical regions of interest in the ATL. N-linked glycans play key roles in protein folding, stability, and function. Biosynthetic modification of N-linked glycans, within the endoplasmic reticulum, features sequential trimming and readornment steps. One unusual enzyme, endo-α-mannosidase, cleaves mannoside linkages internally within an N-linked glycan chain, short circuiting the classical N-glycan biosynthetic pathway. Here, using two bacterial orthologs, we present the first structural and mechanistic dissection of endo-α-mannosidase. Structures solved at resolutions 1.7–2.1 Å reveal a (β/α)8 barrel fold in which the catalytic center is present in a long substrate-binding groove, consistent with cleavage within the N-glycan chain. Enzymatic cleavage of authentic Glc1/3Man9GlcNAc2 yields Glc1/3-Man. Using the bespoke substrate α-Glc-1,3-α-Man fluoride, the enzyme was shown to act with retention of anomeric configuration. Complexes with the established endo-α-mannosidase inhibitor α-Glc-1,3-deoxymannonojirimycin and a newly developed inhibitor, α-Glc-1,3-isofagomine, and with the reducing-end product α-1,2-mannobiose structurally define the -2 to +2 subsites of the enzyme. These structural and mechanistic data provide a foundation upon which to develop new enzyme inhibitors targeting the hijacking of N-glycan synthesis in viral disease and cancer.

In this paper, we propose a novel path-preserving trellis-search (PPTS) algorithm and its high-speed VLSI architecture for soft-output multiple-input-multiple-output (MIMO) detection. We represent the search space of the MIMO signal with an unconstrained trellis, where each node in stage of the trellis maps to a possible complex-valued symbol transmitted by antenna. Based on the trellis model, we convert the soft-output MIMO detection problem into a multiple shortest paths problem subject to the constraint that every trellis node must be covered in this set of paths. The PPTS detector is guaranteed to have soft information for every possible symbol transmitted on every antenna so that the log-likelihood ratio (LLR) for each transmitted data bit can be more accurately formed. Simulation results show that the PPTS algorithm can achieve near-optimal error performance with a low search complexity. The PPTS algorithm is a hardware-friendly data-parallel algorithm because the search operations are evenly distributed among multiple trellis nodes for parallel processing. Conference Paper; Proposed algorithms for Third Generation W-CDMA communication systems have extremely high performance requirements. In this paper, we study the implementation issues involved for one of the proposed multiuser channel estimation and detection algorithms for base-stations in the uplink using TI's TMS320C6x DSP Evaluation Modules(EVM). It was found that these proposed algorithms for multiuser channel estimation and detection have different processing and precision requirements. While the detector can be implemented using the C6201 16-bit fixed point DSP, the proposed channel estimation algorithm may be more suitable for a oating point implementation using the C6701 floating point DSP. We study the effects of the specialized approximate instructions available on the C6701 DSP on channel estimation. Then, the advantage of multistep optimizations and use of assembly code is studied for both the algorithms. Memory issues involved in the implementation of these algorithms is also investigated. It was found that the data memory requirements for channel estimation for the chosen system parameters necessitates the use of external memory while the multistage detection algorithm could be placed in the available internal data memory. In 2001, FYR Macedonia embarked on a new education policy agenda. One of the goals of this agenda was to improve the country's student assessment system. In order to gain a better understanding of the strengths and weaknesses of its existing assessment system, FYR Macedonia decided to benchmark this system using standardized tools developed under The World Bank's Systems Approach for Better Education Results (SABER) program. SABER is an evidence-based program to help countries systematically examine and strengthen the performance of different aspects of their education systems. SABER-student assessment is a component of the SABER program that focuses specifically on benchmarking student assessment policies and systems. The goal of SABER-student assessment is to promote stronger assessment systems that contribute to improved education quality and learning for all. The importance of assessment is linked to its role in: providing information on levels of student learning and achievement in the system; monitoring trends in education quality over time; supporting educators and students with real-time information to improve teaching and learning; and holding stakeholders accountable for results. The SABER-student assessment framework is built on the available evidence base for what an effective assessment system looks like. The framework provides guidance on how countries can build more effective student assessment systems. The Rural Investment Climate Survey (RICS) carried out in 2006 sampled more than 2,500 predominantly very small firms in six rural districts in Indonesia [Labuhan Batu (North Sumatra), Kutai (East Kalimantan), Barru (South Sulawesi), Malang Java), Badung (Bali), Sumbawa (NTB)]. It compiled information on labor force, credit, infrastructure, marketing and competition, the diffusion of technical knowledge, local governance and the main obstacles that firms face. The Indonesian RICS is an in-depth, quantitative survey of 2549 non-farm enterprises, 2782 households and 149 communities in 6 rural districts (Kabupaten). The survey was conducted in January/February 2006 and comprised a linked survey of non-farm enterprises and households. Non-farm enterprises are defined as all enterprises except those undertaking primary agriculture, forestry and fishery activities, although firms which undertake the processing and trading of primary commodities were included. The survey includes both formal and informa businesses and examines both rural and urban areas of rural districts to generate comparative data. This paper concerns one aspect of the work of the Office of Operations Analysis and Research at the Massachusetts General Hospital, namely, its efforts to help meet some of the institution's information needs. A general background is outlined on the development of data processing in hospitals, and on their current needs and problems. The two major computing facilities at the hospital are described. Examples are provided of the contributions of the Office of Operations Analysis and Research in: 1) experimenting with ambulatory care information systems; 2) accessing and preparing analyses from existing data; 3) participating in information systems revision. The ‘computer ombudsman’ roles of this office, some of which are shared with other departments in the medical center, are reviewed.

After ten years of experimentation in computer-assisted cataloging, the Washington University School of Medicine Library has decided to join the Ohio College Library Center network. The history of the library's work preceding this decision is reviewed. The data processing equipment and computers that have permitted librarians to explore different ways of presenting cataloging information are discussed. Certain cataloging processes are facilitated by computer manipulation and printouts, but the intellectual cataloging processes such as descriptive and subject cataloging are not. Networks and shared bibliographic data bases show promise of eliminating the intellectual cataloging for one book by more than one cataloger. It is in this area that future developments can be expected.

Network Function Virtualization (NFV) takes advantage of hardware virtualization to undertake software processing for various functions, and complements the drawbacks of traditional network technology. To speed up NFV related research, we need a user friendly and easy to use research tool, which could support data center simulation, scheduling algorithms implementation and extension, and provide energy consumption simulation. As a cloud simulation toolkit, CloudSim has strong extendibility that could be extended to simulate NFV environment. A direct-detection Brillouin optical time-domain reflectometry (BOTDR) is proposed and demonstrated by using an up-conversion single-photon detector and a fiber Fabry-Perot scanning interferometer (FFP-SI). Taking advantage of high signal-to-noise ratio of the detector and high spectrum resolution of the FFP-SI, the Brillouin spectrum along a polarization maintaining fiber (PMF) is recorded on a multiscaler with a small data size directly. In contrast with conventional BOTDR adopting coherent detection, photon-counting BOTDR is simpler in structure and easier in data processing. In the demonstration experiment, characteristic parameters of the Brillouin spectrum including its power, spectral width and frequency center are analyzed simultaneously along a 10 km PMF at different temperature and stain conditions.; Comment: 4 pages, 5 figures

São apresentados os dados sobre internações por malformações congênitas registradas pelo Centro de Processamento de Dados Hospitalares do Departamento de Medicina Social da Faculdade de Medicina de Ribeirão Preto, USP, desde 19 de janeiro de 1970 a 30 de julho de 1974 em sete estabelecimentos hospitalares. Tais internações participam com 1,48% no total de internações, excluindo-se as causas acidentais. Encontrou-se que as distribuições de diagnósticos principais e secundários classificados por tipo de malformação são significantemente diferentes; descreveu-se a participação de cada estabelecimento nestas internações. Do mesmo modo, encontrou-se um incremento significativo na percentagem de internações por malformações congênitas com o transcurso de tempo, o que é atribuído ao Hospital das Clínicas exclusivamente. Apresentou-se a percentagem de mortalidade por diagnóstico, as razões de sexo e a duração média das internações, encontrando-se nesta última que é maior que a média do total de internações.
