OBJETIVO: Realizar a validação de conteúdo do diagnóstico de enfermagem Angústia espiritual. MÉTODOS: Utilizando o referencial metodológico proposto por Fehring, foi empregado um questionário semi-estruturado, respondido por uma amostra de 72 enfermeiros, para avaliação do título, definição e características definidoras do diagnóstico Angústia espiritual, bem como, sua disposição na Taxonomia II da North American Nursing Diagnosis Association. RESULTADOS: O melhor domínio para classificação do diagnóstico em estudo, segundo os enfermeiros peritos, é o Domínio 10; no entanto, suas classes requerem revisão. Espiritualidade prejudicada demonstrou ser um título adequado ao conceito investigado. A validação das características definidoras identificou sete delas como importantes indicadores clínicos do diagnóstico e apenas a característica não se interessa pela natureza foi considerada pouco relevante. O escore total do diagnóstico foi 0,72, sendo, portanto, considerado validado. CONCLUSÃO: Uma nova proposta apresentada ao fenômeno em estudo foi considerada pertinente. Background: There is little, though growing, interest in the research area of attitudes held among physicians towards disclosing the diagnosis of dementia and Alzheimer`s disease (AD), or the current practice on AD disclosure. This study aimed to investigate the practice and attitudes of specialized physicians towards AD diagnosis disclosure in Brazil. Methods: A questionnaire was devised to survey the current practice and attitudes regarding diagnosis disclosure of AD in Brazil and sent to specialized physicians (170 geriatricians, 300 neurologists and 500 psychiatrists) by electronic mail. Results: From 970 potential respondents, 181 physicians who usually attend AD patients returned the questionnaire. There were no significant differences between the three specialties regarding the frequency with which they informed patients of their AD diagnosis (p = 0.17). The results revealed that only 44.8% of the physicians would regularly inform the patient of the diagnosis, although 85.6% of these use clear terminology. Despite their usual practice, 76.8% would want to know their diagnosis if they themselves were affected by AD. Objective. To identify the factors linked to patients and health services in delays in the diagnosis of tuberculosis. Methods. Epidemiological study in Foz do Iguacu, Parana, Brazil, 2009. The Primary Care Assessment Tool, adapted for appraising tuberculosis treatment, was the instrument used. Descriptive statistics techniques were used, such as frequency distributions, central tendency and dispersion measurements (median and interquartile intervals), and odds ratios. Results. There were greater delays in seeking health services for those in the age group 60 years and older, for females, for patients with low levels of education, and for patients with poor knowledge of the disease. Clinical variables (being a new case and HIV infection) and behavioral variables (use of tobacco and alcohol consumption) were not linked with delays in diagnosis. The median time delays before diagnosis attributable to patients and to the health services were 30 days and 10 days, respectively. Emergency 24-hour medical services and primary health care services were not effective in identifying suspicious cases of tuberculosis and requesting tests to confirm the diagnosis, with a high percentage of referrals to the Tuberculosis Control Program clinic. Este estudo teve como objetivo realizar a revisão do diagnóstico de enfermagem Angústia espiritual proposto pela NANDA (2006); para alcance do mesmo, fundamentou-se nas propostas metodológicas apresentadas por Hoskins e Fehring, sendo desenvolvido em três etapas. Na primeira etapa foi realizada uma revisão integrativa, conduzida pelas questões norteadoras: Qual é a definição de angústia espiritual e quais são os elementos que indicam a presença desse diagnóstico de enfermagem? Utilizando os descritores spirituality e nursing diagnosis, juntamente com o termo spiritual distress, foram localizados 82 estudos, sendo 18 selecionados, os quais estavam indexados nas bases de dados PUBMED, COCHRANE, CINAHL e LILACS e nos North American Nursing Diagnosis Association Conference Proceedings. Os resultados apontaram baixa aplicabilidade desse diagnóstico, revelando a necessidade de reformulação e aperfeiçoamento da proposta apresentada pela NANDA. O estudo teve como proposta analisar se o fenômeno da dor de parto está retratado no diagnóstico de enfermagem Dor Aguda da taxonomia NANDA-I e propor e validar o Diagnóstico Dor de parto. Seguiu-se os preceitos de Hoskins (1989) e Fehring (1986) para as fases da análise de conteúdo e validação clínica do diagnóstico. Na primeira fase foi analisado o conceito de "dor em situação de parto" segundo referencial teórico de Walker e Avant (2005) com sumarização dos dados analisados por meio de revisão integrativa da literatura à estrutura metodológica de Whittemore (2005). A análise do conceito "dor em situação de parto" reforçou a pertinência de se propor um novo diagnóstico e submetê-lo a análise por peritos. O Ecodesign Maturity Model (EcoM2) destina-se a auxiliar as empresas na gestão e aplicação do ecodesign, a partir da seleção de práticas gerenciais a serem incorporadas no PDP e processos relacionados. Esta seleção deriva dos resultados da primeira etapa do seu método de aplicação, o diagnóstico do perfil atual de maturidade em ecodesign. Este diagnóstico caracteriza detalhadamente a empresa em relação a sua capacidade na aplicação das práticas e embasa a criação dos projetos de melhoria, sendo a única forma de avaliação existente no modelo. Outros modelos de maturidade apresentam, além de um diagnóstico detalhado, uma avaliação simplificada. Diagnósticos simplificados elaboram um perfil em alto nível das práticas existentes na empresa em pouco tempo, direcionando investigações detalhadas futuras. OBJECTIVE: To evaluate the discrepancy index between the clinical and histological diagnosis and the prevalence of epithelial dysplasia and carcinoma in 45 patients with potentially malignant epithelial oral lesions (PMEL). PATIENTS AND METHODS: We submitted 45 patients with PMEL to clinical examination and obtained a biopsy from each. The results of histological diagnosis were compared to the clinical diagnosis. RESULTS: Clinical diagnosis showed that the most common PMEL was leukoplakia followed by lichen planus and by actinic cheilitis associated with leukoplakia. The most common site was the buccal mucosa. Histological diagnosis revealed that 46.7% of the PMEL were lichen planus. The discrepancy index between clinical and histological diagnosis was 24.4%. The higher discrepancy index occurred among leukoplakias. The prevalence of epithelial dysplasia and carcinoma was 17.8%. CONCLUSIONS: We conclude that all PMEL should be submitted to a microscopic analysis because the discrepancy between clinical and histological diagnosis was present in a quarter of these lesions. Otherwise, the epithelial dysplasia and carcinoma were more frequent in the leukoplakias.

Regarding the more common allergen sources for animals, fleas, several air-born allergens, as well as many food allergens are frequent causes of allergic reactions, showing different target organs from skin to eye and respiratory or digestive systems. The increasing attention to this field of veterinary clinical pathology needs to run well established guide-lines, either in clinical or in complementary diagnosis. Since Hanifin and Rajka (1980) proposed criteria for atopic dermatitis diagnosis in humans, successive proposals have been also developed to identify atopic dermatitis in dogs. A consensual plan was firstly proposed by Willemse in 1986, undertaking modifications in 1994. In 1998 Prelaud and col. would establish important modifications and in 2009 Favrot proposed several fine tune adjustments, which were supported by the International Task Force on Canine Atopic Dermatitis (ITFCAD) in 2010. Nevertheless, to improve the accuracy of diagnosis, integrating basal knowledge on sensitization etiopathogeny and allergen nature and diversity, allergen sources and implicated molecular allergens for animals should be well identified. OBJECTIVE: To determine the frequency of and the factors related to delayed diagnosis of sarcoidosis in Brazil. METHODS: We evaluated patients with a biopsy-proven diagnosis of sarcoidosis, using a questionnaire that addressed the following: time since symptom onset and since the first medical visit; and the number and specialty of the physicians visited. We divided the patients by the timeliness of the diagnosis-timely (< 6 months) and delayed (≥ 6 months)-comparing the two groups in terms of systemic and pulmonary symptoms; extrathoracic involvement; spirometric data; radiological staging; level of education; income; and tuberculosis (diagnosis and treatment). RESULTS: We evaluated 100 patients. The median number of physicians consulted was 3 (range, 1-14). In 11 cases, sarcoidosis was diagnosed at the first visit. In 54, the first physician seen was a general practitioner. The diagnosis of sarcoidosis was timely in 41 patients and delayed in 59. The groups did not differ in terms of gender; race; type of health insurance; level of education; income; respiratory/systemic symptoms; extrathoracic involvement; and radiological staging. Differential diagnosis is the systematic consideration of the possible cause of signs and symptoms. It requires establishing an anatomic diagnosis, in which the site of pathology is identified, and then an etiiologic diagnosis of the causal pathologic agent. The differential diagnosis is based on a knowledge of the pathophysiology of 1) the presenting signs and symptoms, and 2) the natural history of various diseases and their causative agents. Consequently, the accuracy and reliability of the differential diagnosis is limited by a lack of knowledge about the pathophysiology of various diseases that produce chronic pain. Most painful conditions affecting ectodermally derived tissues are relatively easily localized; the major problems are in the diagnosis of central nervous system pain syndromes and in our knowledge about the pathophysiology of painful neurologic disorders. The anatomic diagnosis of painful diseases of mesodermal and endodermal tissues may be difficult because of the phenomena of referred pain and tenderness. The etiologic diagnosis is made difficult by our lack of knowledge about how various pathologic processes activate nociceptors in these tissues. Finally, the differential diagnosis of chronic pain requires information about the psychological characteristics of the patient and how emotional and higher cognitive functions influence the perception of pain.

Mammography screening is effective in reducing breast cancer (BC) mortality; however there are widespread concerns that it may also lead to over-diagnosis, i.e. the detection of BC that would not have emerged clinically in a woman’s lifetime had she not participated in screening. The extent of over-diagnosis due to mammography is contested, with published estimates varying from 0% to 54%. The principal aim of this research is to quantify the level of over-diagnosis of BC associated with population-based mammography screening in South Australia (SA). The following questions are addressed: (1) Have BC incidence rates increased following the introduction of screening in SA and is the increase greater than expected based on projections of pre-screening trends?, (2) Has the prevalence of key breast cancer risk factors also increased?, (3) To what extent does hormone replacement therapy (HRT) use affect breast cancer risk and screening outcomes?, (4) Are there any differences in the underlying risk of BC among screening participants and non-participants?, and the central question (5) What is the level of over-diagnosis due to organised mammography screening in SA? Introduction: The Familial Amyloid Polyneuropathy, with the world’s largest focus in Portugal, is recognized by the National Board of Assisted Reproductive Technologies as a serious disease eligible for Preimplantation Genetic Diagnosis. This study aims to determine the prevalence of the use of Preimplantation Genetic Diagnosis in FAP carriers followed in Unidade Clínica de Paramiloidose, Centro Hospitalar do Porto, and to identify the associated factors. Material and Methods: Between January and May 2013, a representative sample of Portuguese Familial Amyloid Polyneuropathy carriers, aged between 18 and 55 years, was systematically recruited. The analysis is based on 111 carriers with previous familial diagnosis, who reported having ever tried to get pregnant after 2001. Data on sociodemographic characteristics and use of Preimplan¬tation Genetic Diagnosis were collected through a self-administered questionnaire. Proportions were compared using the chi-square test. Introdução: As cardiopatias congénitas complexas são patologias graves, e o diagnóstico pré-natal poderá ter implicações sobre a morbilidade e a mortalidade. O objetivo deste trabalho foi estudar a influência do diagnóstico pré-natal e da distância do local de parto a um centro de referência, na morbilidade imediata e mortalidade precoce de um grupo de doentes com cardiopatias congénitas complexas.Material e Métodos: Análise retrospetiva dos doentes com cardiopatias congénitas complexas, seguidos no nosso hospital, nascidos entre 2007 e 2012.Resultados: Identificaram-se 126 doentes com cardiopatias congénitas complexas. Em 95% a gravidez foi vigiada desde o primeiro trimestre existindo diagnóstico pré-natal em 42%. Houve relação estatisticamente significativa entre o local do parto e a existência de diagnóstico pré-natal. A cardiopatia congénita complexa mais frequente foi a transposição das grandes artérias (45,2%), seguida da atresia da pulmonar com comunicação interventricular (17,5%) e ventrículo esquerdo hipoplásico (9,5%). Oitenta e dois doentes (65,1%) foram medicados com prostaglandinas e 38 (30,2%) foram ventilados antes de uma intervenção. A cirurgia ocorreu no período neonatal em 73%. The puerperium occurs concomitant to the efetive exercise of maternity, in which a woman experiences profounds changes, within biological and in psychological and socio-cultural. The planned assistance, with the lifting of data, identification of human responses to problems or events from the life cycle in individual aspects, considering the context in which this woman is embedded is crucial during the post-partum phase. Studies with focus on nursing diagnosis (ND) identified in the puerperium have been made eminently at the hospital's context. This survey aimed to analyze the occurrence of 26 diagnosis of nursing care in the interest of immediate and late puerperium in the context of the community. This is a descriptive study of quantitative approach, carried out between February and May 2008. BACKGROUND: Hospitals in countries with public health systems have recently adopted organizational changes to improve efficiency and resource allocation, and reducing inappropriate hospitalizations has been established as an important goal. AIMS: Our goal was to describe the functioning of a Quick Diagnosis Unit in a Spanish public university hospital after evaluating 1,000 consecutive patients. We also aimed to ascertain the degree of satisfaction among Quick Diagnosis Unit patients and the costs of the model compared to conventional hospitalization practices. DESIGN: Observational, descriptive study. METHODS: Our sample comprised 1,000 patients evaluated between November 2008 and January 2010 in the Quick Diagnosis Unit of a tertiary university public hospital in Barcelona. It is currently under an indefinite embargo pending publication by the BMJ Group.; Objective: To examine patient and primary healthcare factors and stage at diagnosis in women with cervical cancer in Northern Uganda with the intention to identify factors that are associated with advanced stages in order to inform policies to improve survival from cervical cancer in the low- and middle-income countries. Design: Cross sectional hospital-based study Setting: Tertiary, not-for-profit private hospital in post-conflict region. Participants: Consecutive tissue-diagnosed symptomatic cervical cancer patients attending care. Of 166 patients, 149 were enrolled and analysed. Primary outcome: Cervical cancer stage at diagnosis. Results: Most women were diagnosed in stages III (45%) or IV (21%). BACKGROUND: Hospitals in countries with public health systems have recently adopted organizational changes to improve efficiency and resource allocation, and reducing inappropriate hospitalizations has been established as an important goal. AIMS: Our goal was to describe the functioning of a Quick Diagnosis Unit in a Spanish public university hospital after evaluating 1,000 consecutive patients. We also aimed to ascertain the degree of satisfaction among Quick Diagnosis Unit patients and the costs of the model compared to conventional hospitalization practices. DESIGN: Observational, descriptive study. METHODS: Our sample comprised 1,000 patients evaluated between November 2008 and January 2010 in the Quick Diagnosis Unit of a tertiary university public hospital in Barcelona. Included patients were those who had potentially severe diseases and would normally require hospital admission for diagnosis but whose general condition allowed outpatient treatment. Advances in semiconductor technology and design automation methods have introduced a new era for electronic products. With design sizes in millions of logic gates and operating frequencies in GHz, defects-per-million rates continue to increase, and defects are manifesting themselves in subtle ways. Traditional test methods are not sufficient to guarantee product quality and diagnostic programs cannot rapidly locate the root cause of failure in large systems. Therefore, there is a need for efficient fault diagnosis methods that can provide quality assurance, accelerate new product release, reduce manufacturing cost, and increase product yield.

This thesis research is focused on fault-insertion test (FIT) and fault diagnosis at the board and system levels. FIT is a promising technique to evaluate system reliability and facilitate fault diagnosis. The error-handling mechanism and system reliability can be assessed in the presence of intentionally inserted faults, and artificial faulty scenarios can be used as references for fault diagnosis. The semiconductor industry continues to relentlessly advance silicon technology scaling into the deep-submicron (DSM) era. High integration levels and structured design methods enable complex systems that can be manufactured in high volume. However, due to increasing integration densities and high operating speeds, subtle manifestation of defects leads to functional failures at the board level. Functional fault diagnosis is, therefore, necessary for board-level product qualification. However, ambiguous diagnosis results can lead to long debug times and wrong repair actions, which significantly increase repair cost and adversely impact yield.

A state-of-the-art diagnosis system involves several key components: (1) design of functional test programs, (2) collection of functional-failure syndromes, (3) building of the diagnosis engine, (4) isolation of root causes, and (5) evaluation of the diagnosis engine. Advances in each of these components can pave the way for a more effective diagnosis system, thus improving diagnosis accuracy and reducing diagnosis time. Machine-learning techniques offer an unprecedented opportunity to develop an automated and adaptive diagnosis system to increase diagnosis accuracy and speed. This dissertation targets all the above components of an advanced diagnosis system by leveraging various machine-learning techniques.
