A avulsão traumática do nervo óptico é evento raro. É relatado um caso de avulsão completa do nervo óptico unilateral após trauma contuso do bulbo ocular sem afetar a integridade de outras estruturas. Ultra-sonografia com sondas B de 10 e 20 MHz se mostrou útil no diagnóstico da lesão, ao passo que a tomografia de órbita não foi conclusiva. O prognóstico visual é reservado, dependente da acuidade visual inicial, que no presente caso foi ausência de percepção luminosa. Não existe tratamento eficaz para lesões completas do nervo óptico, sendo que nenhum tratamento específico foi utilizado neste caso.; Traumatic optic nerve avulsion is a rare event. A case of complete unilateral optic nerve avulsion after blunt ocular trauma without major damage to other ocular structures is reported. B-scan ultrasonography with 10 and 20 MHz probes proved to be helpful in diagnosing the optic nerve lesion, whereas orbital computed tomography was inconclusive. The prognosis is ominous and it is dependent on the initial visual acuity, which was no light perception in this case. There is no efficient treatment for complete optic nerve avulsion, and no specific treatment was performed in this case.

A ecografia se constitui em método importante na avaliação de anormalidades do segmento posterior do globo ocular. OBJETIVO: Comparar as vantagens e desvantagens da sonda de 10 e 20 MHz da ultra-sonografia B-scan, na avaliação de imagens do segmento posterior do globo ocular. MÉTODOS: Pacientes acompanhados no ambulatório do Hospital das Clínicas da USP (HCFMUSP) entre fevereiro de 2006 e abril de 2007 foram submetidos a exame ultra-sonográfico usando o aparelho CINE-Scan® (Quantel Medical Inc.) com sondas de 10 e 20 MHz. Os pacientes foram examinados por três oftalmologistas experientes, em decúbito dorsal, após instilação de colírio de tetracaína a 1%, inicialmente com a sonda sobre as pálpebras e em seguida diretamente em contato com a esclera ou córnea. As imagens obtidas com sondas de 10 e 20 MHz foram comparadas entre si, durante e após a realização do exame. RESULTADOS: A sonda de 20 MHz mostrou-se superior na avaliação da detecção, forma e limites de estruturas do pólo posterior e a de 10 MHz avaliou com mais detalhes o humor vítreo. Não foram encontradas diferenças nas imagens obtidas com exame transpalpebral ou transescleral com a sonda de 10 MHz. PURPOSE: To report abnormalities of retinal nerve fiber layer (RNFL) thickness using Stratus - optical coherence tomography (OCT) in two patients with optic tract lesions. METHODS: Two patients with long standing homonymous hemianopia from optic tract lesions were submitted to a complete neuro-ophthalmic evaluation and to Stratus -optical coherence tomography examination. RESULTS: Both patients revealed diffuse loss of the RNFL at Stratus - OCT in both eyes. In the eyes with the temporal hemianopia, RFNL loss was diffuse but predominantly in the nasal and temporal areas of the optic disc, the classic pattern of band atrophy of the optic nerve. In the eyes with nasal hemianopia RNFL loss could be documented in the superior and inferior quadrants of the optic disc. RNFL loss correlated well with visual field loss and the expected pattern of RNFL loss in optic tract lesions. As afecções que acometem a via óptica anterior podem acarretar perda de fibras das células ganglionares retinianas, gerando alterações na camada de fibras nervosas da retina (CFNR). O objetivo deste trabalho foi o de comparar a capacidade da tomografia de coerência óptica (OCT-1), do OCT 3 (Stratus OCT) e do polarímetro de varredura a laser (GDx) em diferenciar olhos normais de olhos com atrofia em banda do nervo óptico e hemianopsia temporal, assim como, comparar o desempenho de tais tecnologias entre si. Foram estudados 37 indivíduos num total de 46 olhos divididos igualmente em dois grupos. O Stratus OCT, o OCT 1 e o GDx se mostraram capazes de identificar corretamente a redução global (average) da espessura da CFNR dos indivíduos com AB comparados com uma população normal. OBJETIVO: Avaliar a acurácia da biópsia incisional com trépano de 2mm no diagnóstico das tumores palpebrais. Avaliar aeficácia do método nas lesões ulceradas versus não ulceradas; nas lesões marginais da pálpebra versus lesões em outras áreas da pálpebra e na detecção do subtipo histológico do carcinoma basocelular. MÉTODOS: Análise prospectiva das lesões com suspeita de malignidade palpebral no Setor de Plástica Ocular (HC-FMUSP), de maio de 2008 a dezembro de 2011. As lesões foram fotografadas, e suas características clínicas, mensuradas através do softwareImageJ 1.44. O resultado diagnóstico das biópsias pré-operatórias, realizadas de forma padronizada com trépano de 2 mm, foram comparados à peça cirúrgica (padrão-ouro). A avaliação anatomopatológica tanto da biópsia como do padrão-ouro foi realizada de forma mascarada, e os resultados discordantes, revisados por um segundo patologista. As medidas de sensibilidade, especificidade, valor preditivo positivo (VPP) e valor preditivo negativo (VPN), bem como a acurácia e o coeficiente Kappa, foram calculados para avaliar a concordância do trépano de 2 mm em relação ao padrão-ouro. Objective: To describe the normal bony orbital structure of the large fruit-eating bat (Artibeus lituratus) with emphasis on a unique intraorbital bony structure previously not described in the literature. Procedures: The bony anatomy of the orbital cavity was studied on dissected skulls of large fruit-eating bats. The anatomic description of a unique intraorbital spine was made while studying the bony orbit of macerated skulls. Additional observations were made on dissected formalin-fixed whole heads. Both procedures were performed under a stereo dissecting microscope, using ×2-4-magnification. A histologic analysis of soft tissues surrounding this cylindrical bony structure was performed using cross and longitudinal oblique sections from decalcified whole heads, which had been fixed in formalin. Additionally, biometric measurements and a histomorphometric analysis were performed. Results and conclusions: An intraorbital cylindrical osseous structure measuring 3.96 ± 0.68 mm in length and 155.62 ± 14.03 μm in diameter was observed in the large fruit-eating bat (A. lituratus), creating a unique orbital structural design among mammals. We suggest the name optic spine of the alisphenoid bone. The anatomic, biometric and histologic characterization of this element might contribute to a further understanding of the dynamics of bat vision and the sort of factors that influenced evolution of the visual system of microbats. The authors hope that the documentation of this distinctive anatomic feature will also expand the debate about the phylogenetic analysis of the relationship among bat species in the near future. © 2007 American College of Veterinary Ophthalmologists.

OBJECTIVE: To determine the prevalence of ocular findings of the external structures and anterior segment of the eye, detected by biomicroscopic examination in schoolchildren in Natal (RN) - Brazil. METHODS: After previous random selection, 1,024 pupils from elementary and secondary public and private schools in the city of Natal were evaluated from March to June 2001. All were submitted to preestablished standard research norms, consisting of identification, demographic information, ophthalmologic biomicroscopic examination, with slit lamp, performed by ophthalmologists from the "Onofre Lopes" University Hospital. RESULTS: Alterations of the conjunctival and palpebral conditions were the most prevalent (10.4% and 6.2% respectively). Follicles (4.2%) and papillae (3.0%) were the frequent conjunctival lesions, while blepharitis (3.5%) and meibomitis (1.1%) were the most detected abnormalities in the eyelids. Upon examining the cornea, iris, lens and anterior vitreous, the most encountered findings were nubecula (0.5%), papillary membrane reliquiae (0.5%), posterior capsula opacity (0.8%) and hyaloid arteria reliquiae (2.0%). PURPOSE: To determine the applicability of the international revised diagnostic criteria for Vogt-Koyanagi-Harada disease. METHODS: Retrospective study. Medical charts of 140 patients with the diagnosis of Vogt-Koyanagi-Harada disease, from the Uveitis Sector of the Federal University of Sao Paulo (UNIFESP), were revised and classified following the revised diagnostic criteria. RESULTS: Of the 140 patients, 12.85% fulfilled the criteria for complete disease, 29.28% incomplete disease, 28.57% "probable" Vogt-Koyanagi-Harada disease and 28.27% were considered not Vogt-Koyanagi-Harada disease. CONCLUSION: The authors consider that the international revised diagnostic criteria have good applicability and are very useful to help in the diagnosis of Vogt-Koyanagi-Harada disease.

Hereditary spastic paraplegia (HSP) is characterized by weakness and spasticity of the lower extremities. Kjellin’s syndrome is a rare syndrome associated with HSP. The syndrome is characterized by the presence of bilateral retinal flecks, similar to the findings in Stargardt disease and fundus flavimaculatus. We report the case of a 34-year-old male who presented with complete features of Kjellin’s syndrome, with typical retinal findings observed on multimodal imaging (spectral domain optical coherence tomography [SD-OCT], near-infrared reflectance and autofluorescence imaging). The ophthalmological changes at early stages of the disease may not impair visual acuity. Therefore, the detection of central retinal degeneration requires thorough fundus examination.

Impression cytology (IC) has been widely used as a method for evaluating the ocular surface and superficial cells layers in the diagnosis and follow-up after treatment of several ocular surface tumors of both epithelial and melanocytic origin. Information regarding this can be found in the English-language literature since 1992. Using either cellulose acetate or Biopore membranes for specimen collection, a high correlation has been found between IC and tissue histology. Compared with exfoliative cytology with spatula, IC is less traumatic to the patient’s eye, provides a precise location of the area being studied, and allows accurate observation of the cells the way they exist in vivo. The additional advantage of IC is the preservation of limbal stem cells responsible for continuous corneal epithelium renewal; these can be affected after incisional or excisional biopsy at the corneoscleral limbus, which is the most frequent site of appearance of tumors in the stratified epithelium. Treatment for ocular surface squamous neoplasia has historically included surgery, but nonsurgical interventions have also been adopted. Hence, in certain cases, ophthalmologists may prefer interventions less invasive than surgical biopsy such as of impression cytology for both initial diagnosis and therapeutic monitoring of treatment for ocular surface lesions. PURPOSE: The aim of the present study is to assess central corneal edema and visual recovery after cataract surgery performed according to two technologies: conventional ultrasonic and liquefaction (Aqualase®). METHODS:This is a prospective contralateral study in wich 20 patients with comparable preoperative conditions were submitted to cataract surgery were evaluated. Preoperative assessment involved complete ophthalmological examination and the study included patients with bilateral cataracts up to grade 2, according to the Lens Opacity Classification System II. The same cristaline fracture technique was used in all cases, and surgical procedures were performed by the same experienced surgeon, using two technologies: liquefaction or conventional phacoemulsification. Postoperative central corneal edema was measured by corneal optical pachymetry (Orbscan II®) on the 1st, 3rd, 7th and 10th postoperative days. RESULTS:None of the 20 patients submitted to surgery was lost during the postoperative follow-up or excluded from the analysis. On the first postoperative, the visual acuity average was 0.031 logMAR in the Aqualase® group and 0.043 logMAR in the conventional surgery group. No statistical difference was detected in the assessment of visual acuity throughout the postoperative period. Diagnostic incisional biopsies in clinically indeterminate choroidal tumours

Most intraocular tumours are reliably diagnosed by a careful clinical examination combined with one or more non-invasive diagnostic techniques. However, in a small percentage of tumours, typically small and clinically amelanotic, the features are insufficiently distinct for a confident clinical diagnosis and tissue is required for diagnosis. We used a 23-G vitreous cutter to access the biopsy site in 43 patients with clinically indeterminate tumours. After retinotomy, an incisional choroidal biopsy yielded a specimen of ∼1 mm3. Obtained tissue was routinely processed for light microscopy including an immunohistochemical panel of monoclonal antibodies. Adequate tissue for diagnosis was provided in 41/43 (95%) patients. The sensitivity and specificity to detect malignant disease were 0.97 and 1.00, respectively. The positive predictive value was 1.00. Complications included progression of pre-existing retinal detachment in 5/43 (12%) patients and transient rise in intraocular pressure to >40 mm Hg in 6/43 (14%) patients; 4 of these 6 patients had a pre-existing retinal detachment. No patient with a pre-operatively attached retina had a retinal detachment. We conclude that an incisional transretinal choroidal biopsy yields abundant material and may adequately confirm or exclude malignancy in patients with clinically indeterminate tumours. The complication rate can be minimised when patients with pre-existing retinal detachment are excluded from biopsy.

Developments in fiberoptic technology and increasing interest in minimally invasive surgery have fueled advances in transcanalicular surgery. This article presents a review of instruments and methods for diagnostic and therapeutic approaches to adult lacrimal drainage obstruction. Available endocanalicular probes, microendoscopes, lasers, microdrills, trephines, and antegrade lacrimal balloon catheters are discussed and compared. Developments in microendoscopy, laser transcanalicular dacryocystorhinostomy, laser canaliculoplasty, transcanalicular drilling and trephination, and transcanalicular balloon dacryoplasty are also discussed in detail. Transcanalicular surgery provides a minimally invasive approach to adult lacrimal drainage obstruction that may also address the pathology causing the obstruction. Long-term success rates of transcanalicular dacryocystorhinostomy appear to be improving, but cost and a paucity of data on long-term results continue to limit the use of transcanalicular surgery.; Paul A. Objective To perform a multicenter review of the clinical features and treatment of 31 patients with idiopathic sclerosing orbital inflammation. Methods We included all patients with histologically confirmed idiopathic sclerosing orbital inflammation from 5 regional orbital centers. We reviewed the case notes to determine the clinical presentation, diagnostic features, and response to treatment. The main outcome measures were duration and nature of symptoms, anatomical location of disease, histopathological findings, treatment modalities, treatment efficacy and adverse effects, and final clinical status. Results We included 13 male and 18 female patients ranging in age from 7 to 83 years. The average duration of symptoms at presentation was 13.4 months. There was a predilection for the lateral and superior quadrants. Thirteen patients had apical disease, and 4 had extraorbital involvement. Histopathological findings invariably showed sclerosis associated with a sparse mixed cellular infiltrate. Twenty-seven patients were treated with oral prednisolone, response to which was good in 9 patients, partial in 11, and poor in 7. Six patients were treated with a second-line immunosuppressive agent, and 6 received radiotherapy. The response to radiotherapy was generally poor. Conclusions Idiopathic sclerosing orbital inflammation is a rare condition that can be difficult to diagnose and manage. PURPOSE: To evaluate the reproducibility of peripapillary retinal nerve fiber layer (RNFL) thickness measurements in normal eyes and eyes with glaucoma using spectral domain optical coherence tomography (SDOCT). METHODS: One eye of 79 normal and 72 glaucoma patients was analyzed. All patients underwent a complete ophthalmological examination, including visual acuity testing; intraocular pressure, slit-lamp examination, indirect ophthalmoscopy; and the glaucoma group underwent achromatic perimetry with the 24-2 SITA Fast Humphrey Field Analyzer. All patients' eyes were scanned using the spectral domain optical coherence tomography - Spectralis® and one of them was chosen randomly. Three consecutive circular B-scan centered at the optic disc were performed in one visit. RESULTS: The intraclass correlation coefficient (ICC), coefficient of variation and test-retest variability for the mean retinal nerve fiber layer thickness were respectively: 0.94, 2.56% and 4.85 µm for the normal group and 0.93, 4.65% and 6.61 µm for the glaucomatous group. The intraclass correlation coefficient for retinal nerve fiber layer thickness in all quadrants were all excellent in both groups, with the superior quadrant having the highest ICCs (0.964) in glaucomatous eyes and nasal quadrant measurements having the lowest (0.800)...

Impression cytology features of conjunctival nevi reported as more noticeable

PURPOSE: To report the impression cytology features of conjunctival nevi reported as more noticeable. METHODS: 35 patients who reported that a conjunctival lesion had become more noticeable after color or size change were enrolled. On slit-lamp examination, a clinical diagnosis of nevus was made and lesions underwent impression cytology using acetate cellulose strips and a combined staining with PAS, H&E and Papanicolaou. At patient's or parents' request, excision of the lesion was performed and the tissue was submitted to histopathological study. RESULTS: Impression cytology examination revealed nests or cluster of nevus cells within the epithelium layer containing or not mucous-secreting goblet cells in 32 cases (91.4%). Ten patients (28.5%) had the tumor removed and histopathological diagnosis was compound nevus in 8 eyes (1 from caruncle, 1 from plica semilunaris and 6 from bulbar conjunctiva) and subepithelial nevus from bulbar conjunctiva (2 eyes). CONCLUSION: Optical microscopy analysis of the impression cytology specimens confirmed the clinical diagnosis by demonstrating typical histopathological features of the superficial layers from conjunctival nevi in 91.4% of the cases. For amelanotic nevi IC can also allow a differential diagnosis from other nonpigmented lesions. The technique does not replace histopathological examination...

PURPOSE: To evaluate the sensitivity and specificity of machine learning classifiers (MLCs) for glaucoma diagnosis using Spectral Domain OCT (SD-OCT) and standard automated perimetry (SAP). METHODS: Observational cross-sectional study. Sixty two glaucoma patients and 48 healthy individuals were included. All patients underwent a complete ophthalmologic examination, achromatic standard automated perimetry (SAP) and retinal nerve fiber layer (RNFL) imaging with SD-OCT (Cirrus HD-OCT; Carl Zeiss Meditec Inc., Dublin, California). Receiver operating characteristic (ROC) curves were obtained for all SD-OCT parameters and global indices of SAP. Subsequently, the following MLCs were tested using parameters from the SD-OCT and SAP: Bagging (BAG), Naive-Bayes (NB), Multilayer Perceptron (MLP), Radial Basis Function (RBF), Random Forest (RAN), Ensemble Selection (ENS), Classification Tree (CTREE), Ada Boost M1(ADA),Support Vector Machine Linear (SVML) and Support Vector Machine Gaussian (SVMG). Areas under the receiver operating characteristic curves (aROC) obtained for isolated SAP and OCT parameters were compared with MLCs using OCT+SAP data. RESULTS: Combining OCT and SAP data, MLCs' aROCs varied from 0.777(CTREE) to 0.946 (RAN).The best OCT+SAP aROC obtained with RAN (0.946) was significantly larger the best single OCT parameter (p<0.05)...

Amelanotic corneally displaced malignant conjunctival melanoma: a case report evaluated with impression cytology

Here we describe the case of a 65-year-old Caucasian female who presented with an amelanotic malignant conjunctival melanoma and highlight the clinical and pathological features of this rare entity that displayed exclusive corneal invasive growth without evidence of conjunctival tumors other than primary acquired melanosis. Impression cytology aided in the initial diagnosis. The patient underwent surgical treatment. Histopathology and immunohistochemistry revealed an invasive amelanotic melanoma limited to the cornea and exhibiting S-100, Melan A, and HMB-45 positivity. The absence of pigmentation delayed early clinical detection and treatment. Awareness of this nonpigmented melanoma is important for early recognition and appropriate management.

Introdução: Entre os acidentes automobilísticos que ocorrem em vias públicas, uma parte também pode ser considerada acidentes de trabalho, haja vista que a maioria dos envolvidos se encontra a serviço das empresas. O principal sistema orgânico utilizado no ato de dirigir é o sentido da visão, devendo este ser bem avaliado na obtenção e renovação da habilitação de motoristas profissionais. As estatísticas brasileiras apontam para um aumento global do número de óbitos por acidentes de trânsito, especialmente entre motoristas profissionais de veículos de grande porte entre 1998 e 2008. Em comparação com os Estados Unidos, o Brasil apresenta um maior índice global de óbitos no trânsito. Pela importância social e econômica desse grupo de motoristas, faz-se necessária a realização de uma avaliação oftalmológica mais estrita que na população em geral. Objetivos: Discutir sobre as diferenças entre os índices mínimos exigidos no exame oftalmológico para obtenção e renovação da carteira de habilitação profissional no Brasil e propor aprimoramento das técnicas de diagnóstico oftalmológico em motoristas profissionais no Brasil. Materiais e Métodos: Foi realizada uma revisão bibliográfica nas bases de dados Pubmed e SciELO em que foram selecionados os artigos publicados em português e inglês entre 1980 e 2011. As palavras-chave utilizadas foram: Técnicas de diagnóstico oftalmológico (Ophthalmological diagnostic techniques)...
