Effect of postnatal malnutrition on hyperoxia-induced newborn lung development

Several factors are associated with bronchopulmonary dysplasia. Among them, hyperoxia and lung immaturity are considered to be fundamental; however, the effect of malnutrition is unknown. Our objective was to evaluate the effects of 7 days of postnatal malnutrition and hyperoxia on lung weight, volume, water content, and pulmonary morphometry of premature rabbits. After c-section, 28-day-old New Zealand white rabbits were randomized into four groups: control diet and room air (CA, N = 17), control diet and ≥95% O2 (CH, N = 17), malnutrition and room air (MA, N = 18), and malnutrition and ≥95% O2 (MH, N = 18). Malnutrition was defined as a 30% reduction of all the nutrients provided in the control diet. Treatments were maintained for 7 days, after which histological and morphometric analyses were conducted. Lung slices were stained with hematoxylin-eosin, modified orcein-resorcin or picrosirius. The results of morphometric analysis indicated that postnatal malnutrition decreased lung weight (CA: 0.83 ± 0.19; CH: 0.96 ± 0.28; MA: 0.65 ± 0.17; MH: 0.79 ± 0.22 g) and water content, as well as the number of alveoli (CA: 12.43 ± 3.07; CH: 8.85 ± 1.46; MA: 7.33 ± 0.88; MH: 6.36 ± 1.53 x 10-3/mm) and elastic and collagen fibers. Background: In Brazil hospital malnutrition is highly prevalent. physician awareness of malnutrition is low, and nutrition therapy is underprescribed. One alternative to approach this problem is to educate health care providers in clinical nutrition. The present study aims to evaluate the effect of an intensive education course given to health care professionals and students on the diagnosis ability concerning to hospital malnutrition. Materials and methods: An intervention study based on a clinical nutrition educational program, offered to medical and nursing students and professionals, was held in a hospital of the Amazon region. Participants were evaluated through improvement of diagnostic ability, according to agreement of malnutrition diagnosis using Subjective Global Assessment before and after the workshop, as compared to independent evaluations (Kappa Index, k). To evaluate the impact of the educational intervention on the hospital malnutrition diagnosis, medical records were reviewed for documentation of parameters associated with nutritional status of in-patients. The SPSS statistical software package was used for data analysis. Results: A total of 165 participants concluded the program. The majority (76.4%) were medical and nursing students. O trabalho estudou o estado nutricional de uma amostra aleatória (n = 98) de idosos vivendo na zona urbana do Distrito Bonfim Paulista e procurou identificar alguns fatores relacionados ao risco de desnutrição. Os idosos selecionados foram avaliados quanto ao estado nutricional através da Mini Avaliação Nutricional (MAN) e através do recordatório alimentar de 24 horas. Também foi elaborado um questionário domiciliar especifico contendo questões relacionadas à escolaridade, renda do idoso e da família, perfil multidimensional, impressão subjetiva de saúde e de qualidade de vida, atividade de trabalho, moradia, doenças crônicas, atividade de vida diária e participação social. Procuramos, desta forma, identificar a prevalência de desnutrição e risco de desnutrição na comunidade e identificar fatores associados com a menor pontuação na MAN, ou seja, fatores que possam contribuir para um pior prognóstico nutricional. Participaram do trabalho 98 voluntários sendo 30 (30,6%) idosos do sexo masculino e 68 (69,4%) do sexo feminino. Detectamos a prevalência de 11,2% de idosos desnutridos, 37,7% de idosos sob risco de desnutrição e 51,1% de bem nutridos de acordo com MAN. Objetivo. A desnutrição em indivíduos hospitalizados tem apresentado elevada prevalência no Brasil e no mundo e é conseqüência de uma série de fatores. O presente trabalho teve por objetivo estudar os fatores associados ao risco de desnutrição em adultos hospitalizados. Casuística e Métodos. Foi um estudo transversal e a amostra constituída por 300 indivíduos hospitalizados com idade de 18 a 64 anos, sorteados na listagem de internação das ultimas 48 horas. Foram conduzidos dois modelos de regressão logística múltipla, adotando-se a desnutrição como variável dependente. O diagnóstico do estado nutricional foi obtido com uma avaliação nutricional completa. No primeiro modelo, foram selecionadas e incluídas todas variáveis de interesse e que apresentaram nível de significância de 20por cento (p<0,20). No segundo modelo, as variáveis mais fortemente associadas no primeiro foram retiradas com o objetivo de avaliar outros fatores. A partir das variáveis estudadas, foram desenvolvidos e propostos dois instrumentos de triagem nutricional. Resultados. Inúmeros estudos têm demonstrado que a desnutrição protéica e/ou protéico-calórica, quando introduzida durante os estágios iniciais de desenvolvimento, produz diversas alterações neuroquímicas, neurofisiológicas e/ou funcionais no sistema nervoso central, além de comprometimentos físicos e atrasos no desenvolvimento cognitivo. O presente trabalho teve como objetivo investigar os efeitos de quatro técnicas de desnutrição precoce sobre os processos de aprendizagem e memória espacial (operacional e de referência) em ratos adultos testados em duas tarefas de aprendizagem em dois modelos de labirintos aquáticos. It is estimated that in developing countries, about 30% of the population of
children under 5 years of age are malnourished. According to UNICEF the causes of
malnutrition include other factors besides availability of food and family violence
have been cited as a risk factor for child malnutrition. In order to check whether there
is an association between violence and moderate or severe malnutrition in children it
was conducted a case-control study in a community of the city of Maceió, Alagoas.
Cases were 65 pairs of mother malnourished child (children with moderate or
severe malnutrition, aged between six and sixty months of age), attended at the
Center for Nutritional Rehabilitation and Education (CREN) - Maceió, and controls
were 64 pairs of mother- eutrophic child in the same age and without malnourished
siblings. We used the questionnaire Brazilian version of the WorldSAFE to detect
violence against women and against children and anthropometric evaluation to
detect child malnutrition. Data were analyzed utilizing the chi square test and the
odds ratio (OR). It was utilizaed the value of p ≤ 0.05 and confidence interval of
95%. The results showed that both severe physical violence against children by
parents, and as physical violence against women by the husband or partner in the

Cette étude s’inscrit dans le cadre du projet « Pôle francophone africain sur le Double Fardeau Nutritionnel » (DFN) du laboratoire TRANSNUT, centre collaborateur OMS sur la transition nutritionnelle du Département de Nutrition de l’Université de Montréal, en collaboration avec ses partenaires au Burkina Faso, à savoir l’Institut de Recherche en Sciences de la Santé (IRSS) et l’Institut Supérieur des Sciences de la Population (ISSP). Elle est l’une des premières à s’intéresser au double fardeau de la malnutrition en Afrique francophone. Cette étude avait pour objectif de démontrer l’existence du double fardeau de la malnutrition parmi les adultes de Ouagadougou au Burkina Faso, d’en donner l’ampleur et d’identifier ses liens avec les facteurs du mode de vie. Plus spécifiquement, elle visait à décrire les carences nutritionnelles, les facteurs de risque cardiométabolique (FRCM), et la typologie du double fardeau de la malnutrition; examiner les caractéristiques du mode de vie des adultes et leurs liens avec le double fardeau de la malnutrition selon les conditions économique, et enfin d’examiner l’association entre inflammation subclinique, les carences nutritionnelles et les FRCM selon les facteurs du mode de vie. childhood, and, it increases the risk of chronic diseases in
later life. The Millennium Development Goals cannot be
reached without significant efforts to eliminate
malnutrition. The book looks at ways to combat malnutrition,
by positioning nutrition directly on the poverty and human
development policy agenda, to ensure large-scale nutrition
actions, and develop capacity to address malnutrition. It

of malnutrition. Malnutrition in childhood is the biggest
contributor to child mortality; a third of child deaths have

The objective of this paper is to
document the extent, and distribution of child, and adult
malnutrition in Guatemala; to analyze the relationship
between selected child, maternal, household and community
characteristics, and children's nutritional status;
and, to outline the implications of the most important
findings for nutritional policy. The prevalence of chronic
malnutrition among Guatemalan children in 2000, was the
highest in Latin America, and among the highest in the
world. The data show very strong socioeconomic, and
geographic inequality. The econometric analysis reveals a
strong impact of income, and of inter-generational effects.
Education of adults in the household, and the availability
of infrastructure, are other important determinants of
children's growth attainment. Finally, even controlling
for income, and other household and community
characteristics, ethnicity remains an important determinant

Ethiopia has one of the highest child malnutrition rates in the world. A considerable effort to monitor child malnutrition rates over the last two decades shows that, despite some improvements, approximately half of the children under five are still malnourished. Much of the burden of deaths resulting from malnutrition, estimated to be over half of childhood deaths in developing countries, can be attributed to just mild and moderate malnutrition. Several biological and social economic factors contribute to malnutrition. Using the 2000 Ethiopia Demographic and Health Survey data, this study examines the impact of access to basic environmental services, such as water and sanitation on the probability children are stunted and underweight. The focus is on the impact of externalities associated with access to these services. We find that biological factors, such as child's age and mother's height, and social economic factors, such as household wealth and mother's education, are important determinants of a child's nutritional status. This is consistent with the findings of most studies in this literature. With respect to the environmental factors, we find that there are indeed significant externalities associated with access to water and sanitation at the community level. The external impacts of community level of access to these services are an important determinant of the probability a child is underweight. Our results also show that the external impact of access to water is larger for children living in rural areas.

O Programa Fome Zero é um dos itens mais importantes da agenda social do governo atual. Apesar da enorme atenção que ele tem obtido na mídia e seu conteúdo ser de interesse dos profissionais de saúde pública, poucos sanitaristas têm-se manifestado a seu respeito. O objetivo deste artigo é rever os conceitos do Programa, sob a perspectiva dos epidemiologistas, observando o uso dos conceitos pobreza, fome e desnutrição na literatura biomédica. Ainda, através do estudo da distribuição da mortalidade por desnutrição, procura-se identificar as populações vulneráveis à desnutrição, quanto a: idade, alfabetização e localização geográfica. Apresenta-se, também, um quadro de referência para o entendimento das relações entre a pobreza e a desnutrição. Introducción: La desnutrición es un problema de salud muy frecuente en la población con insuficiencia renal crónica en diálisis. Dada la alta prevalencia de malnutrición en los pacientes en diálisis y su repercusión sobre la morbi-mortalidad de los pacientes es preciso instaurar un adecuado seguimiento de los aspectos nutricionales, para detectar precozmente a pacientes en riesgo o con déficit nutricionales y realizar una intervención nutricional precoz que consiga revertir la situación. Rationale: Disease-related malnutrition constitutes a highly prevalent healthcare problem with high costs associated. In Spain, the prevalence of malnutrition in hospitalized patients has been reported from 30% to 50%. Objectives: Main purposes of this consensus document were to establish recommendations that facilitate decision-making and action to prevent and early-diagnose disease-related hospital malnutrition, on the management of nutritional support methods and actions to evaluate nutritional treatment compliance and efficacy. Methods: A systematic bibliographical search of authors was performed, complemented by updated bibliography by author references up to 2010. From this review, some recommendations were defined, modified and critically evaluated by the representatives of scientific societies in a consensus conference (Dec 2010) following a structured brainstorming technique: the Metaplan® technique. A double validation process was undertaken until final recommendations were obtained. Results: 30 consensus recommendations for the prevention and management of hospital malnutrition are presented in this document. Recommendations cover all clinical care settings as well as prevention, screening, diagnosis, treatment and follow-up of disease-related malnutrition. Conclusions: Nutritional screening is strongly recommended at all clinical settings when nutritional risk factors are identified or there is clinical suspicion of malnutrition. Background: Malnutrition among inpatients is highly prevalent, and has a negative impact on their clinical outcome. The Working Group for the Study of Malnutrition in Hospitals in Catalonia was created to generate consensus guidelines for the prevention and/or treatment of malnutrition in hospitals in Catalonia, Spain. Aims: The objectives of the study were to determine the prevalence of malnutrition on admission to hospital in Catalonia and to assess relationships between malnutrition, social and demographic data, overall costs, and mortality. Methods: Prospective and multicenter study conducted with 796 patients from 11 hospitals representative of the hospitalized population in Catalonia. Nutritional status was evaluated using the Nutritional Risk Screening 2002 method. Results: Overall, 28.9% of the patients are malnourished or at nutritional risk. Elderly patients, non-manual workers, those admitted to hospital as emergencies and with higher co-morbidities had higher risk of malnutrition. The type of hospital (second level vs. tertiary or University referral) to which they were admitted was also a factor predisposing to malnutrition. Length of hospital stay was longer in malnourished patients (10.5 vs. 7.7 days, p < 0.0001). Background and aims: The main objective of the PREDyCES® study was twofold. First, to analyse the prevalence of hospital malnutrition in Spain, both at admission and at discharge, and second, to estimate the hospital costs associated with disease-related malnutrition. Methods: The study was a nationwide, cross-sectional, observational, multicentre study in routine clinical practice, which assessed the prevalence of hospital malnutrition both at patient admission and discharge using NRS-2002®. A study extension analysed the incidence of complications associated with malnutrition, excess hospital stay and healthcare costs associated with hospital malnutrition. Results: Malnutrition was observed in 23.7% of patients according to NRS-2002®. Multivariate analysis revealed that age, gender, presence of malignant disease, diabetes mellitus, dysphagia and polymedication were the main factors associated with the presence of malnutrition. Malnutrition was associated with an increase in length of hospital stay, especially in patients admitted without malnutrition but who presented malnutrition at discharge (15.2 vs. 8.0 days, p < 0.001), with an associated additional cost of €5,829 per patient. Background & aims: Malnutrition prevalence is unknown among elderly patients with diabetes mellitus. Our objectives were to determine malnutrition prevalence in elderly in patients with diabetes, and to describe their impact on prognosis. Methods: An observational multicenter study was conducted in 35 Spanish hospitals. Malnutrition was assessed with the Mini Nutritional Assessment (MNA) tool. Patients were followed until discharge. Results: 1,090 subjects were included (78 ± 7.1 years; 50% males). 39.1% had risk of malnutrition, and 21.2% malnutrition. A 15.5% of the malnourished subjects and 31.9 % of those at risk had a BMI ≥ 30 kg/m². In multivariate analysis, female gender (OR = 1.38; 95% CI: 1.19-1.11), age (OR = 1.04; 95% CI: 1.02-1.06) and presence of diabetic complications (OR = 1.97; 95% CI: 1.52-2.56) were associated with malnutrition. Length of stay (LOS) was longer in at-risk and malnourished patients than in well-nourished (12.7 ± 9.9 and 15.7 ± 12.8 days vs 10.7 ± 9.9 days; p < 0.0001). After adjustment by age and gender, MNA score (OR = 0.895; 95% CI 0.814-0.985) and albumin (OR = 0.441; 95% CI 0.212-0.915) were associated with mortality.
