Narcolepsia é uma doença neurológica crônica caracterizada por sonolência diurna excessiva e ataques de sono. Ocorre na população de uma para cada mil pessoas. Fenômenos de sono REM (rapid eye movement), cataplexia, paralisia do sono e alucinações hipnagógicas podem também ocorrer. Afeta todos os aspectos da vida e pode causar dificuldades para os pacientes em manter seus empregos, nos relacionamentos interpessoais e riscos de acidentes devido à sonolência excessiva e à cataplexia. Acredita-se que seja causada por um interjogo de fatores genéticos e ambientais. O risco para familiares de primeiro grau é estimado em de 1-2%. Recentemente foi demonstrado que os pacientes com narcolepsia têm uma deficiência de hipocretina também chamada de orexina na parte lateral do hipotálamo. O presente trabalho teve como objetivos: a) caracterizar os aspectos psicodinâmicos mais freqüentes da amostra; b) verificar a eficácia adaptativa do ego; c) verificar o equilíbrio e o funcionamento psíquico e relacionar com aspectos clínicos dos pacientes. Background: Scleroderma is a chronic disorder, characterized by thickening and fibrosis of the skin and disruption of internal organs that affects global functioning and quality of life. Facial skin alterations have an impact on self-esteem and interfere with social interactions, with an association to psychological symptoms, especially depression. The aim of this study, in collaboration with a Canadian team and European Associations, intended to characterize the symptoms and the psychological problems in European patients with scleroderma. Methods: 563 participants from 13 European Countries; 484 (89%) female; 59 (11%) male; mean age: 49 (SD= 14.8); 93% caucasian; 68% married; 32% higher education filled out the Patient Survey of Health Concerns and Research Priorities. Objectives: this study assessed burden, coping, physical symptoms and psychological morbidity in caregivers of functionally dependent family members. Methods: fifty family caregivers completed self-reported measures of burden, physical symptoms, psychological morbidity and coping strategies. Results: there was a significant negative correlation between coping strategies and the different clinical variables, as well as a significant positive correlation between coping strategies and duration of care. It appears that the stronger bond between caregiver and family member leads to a poorer use of adaptive coping strategies. It also appears that the deterioration of the relationship between them and the lower perceived self-efficacy are more prominente in caregivers of family members with cognitive impairment, indicating that caregivers with family members without cognitive impairment face fewer difficulties. Conclusion: these results emphasize the need for interventions to include coping strategies, since they are important in reducing caregivers’ burden, psychological morbidity and physical symptoms.

OBJECTIVES: To investigate coping strategies used by patients submitted to heart transplantation and whether they are related to the perception of the disease and transplantation. METHODS: Cross-sectional study with quantitative and qualitative analysis. The sample of 32 patients was assessed by the Ways of Coping Scale and socio-demographic questionnaire, and five of them were selected for interviews. The internal consistency of the scale was assessed, the variables and strategies involved were crossed and content analysis of interviews, investigating the existence of a relationship with the speech of the participants. RESULTS: The individuals have used all coping styles, with a predominance of the problem-focused strategy. Psychologically prepared patients showed a statistically significant increase in the use of problem-focused coping and seek for social support. However, a significant increase in the use of emotion-focused coping was observed in patients who were not prepared. Analysis through the method of Bardin showed as categories: disease; reaction to call; transplantation; fantasies; postoperative; team and coping. CONCLUSION: Patients with a transplanted heart make use of all coping strategies, with a predominance of the problem-focused strategy. Malgré des recherches intensives portant sur l’hérédité et les aspects biologiques de la rétinite pigmentaire (RP), peu de recherches fondées ont porté sur les aspects psychologiques. Ces quelques études suggèrent que les personnes atteintes de rétinite pigmentaire s’adaptent différemment à la déficience visuelle. Le but de la présente étude était donc de vérifier si les personnes atteintes de rétinite pigmentaire s’adaptaient différemment d’un point de vue psychologique par rapport à des personnes ayant une déficience visuelle causée par une autre pathologie. Des entrevues téléphoniques incluant des personnes ayant la rétinite pigmentaire, la rétinopathie diabétique (RD) et l’albinisme ont été menées. The adaptation to a new country is a complex and stressful process that is compounded when changes in status and identity have to be made. This exploratory study examined the adaptation of international company transferee spouses when they decide to follow the transferee on overseas assignments. Research to date indicates that the spouses’ dissatisfaction with life abroad is the leading cause of transferees breaking contract and prematurely returning home. The causes of this dissatisfaction are still not clear and this study sought greater clarification, particularly examining the experiences of male as well as female trailing spouses. The study, thus, takes gender as a main variable to consider. It explores how gendered expectations inherent in the structures of society inflect and inform the decisions, attitudes, and behaviors that affect the adaptations of trailing spouses living in a foreign habitus. The study is based on eight months of ethnographic research in two culturally different locations, Kuala Lumpur, Malaysia and Brussels, Belgium. OBJECTIVE: The present study addressed a National Health and Medical Research Council (NHMRC) recommendation that the outcome of neonates who are treated conventionally for respiratory problems be further investigated before setting up additional extracorporeal membrane oxygenation (ECMO) centres in Australia. METHODOLOGY: The cognitive and behavioural outcome of ECMO eligible infants who received conventional treatment for respiratory problems at birth was assessed in 18 infants aged 1.5-3 years (index group). ECMO was not available at either of the treating hospitals. Index children were compared to a matched control group of children who did not experience any major complications at birth. Children were assessed using either the Bayley or McCarthy scales of infant development and the Child Behavior Checklist. RESULTS: Overall, a mortality rate of 19% and a psychological morbidity rate of 18% suggest that children born with severe respiratory failure, who meet existing ECMO eligibility criteria, have a good prognosis when treated using conventional medical therapy. CONCLUSIONS: Improvements to conventional treatments indicate that ECMO eligibility criteria may need to be revised to identify accurately those infants who are at extreme risk of mortality if treated conventionally.

Recent literature has been concerned with the relationship between life-events and psychological symptoms, and more particularly studies of communities subsequent to disasters have suggested the presence of intrusive cognitions to be a mediating variable. In this investigation new patients presenting to a general community psychiatric clinic over a four-month period completed a brief life-event scale, the Impact of Event Scale (IES) and the 28-item General Health Questionnaire (GHQ). Appropriate statistical methods were employed to model the intervening role of IES scores between numbers of life-events and subsequent psychiatric symptomatology. Data was collected on 48 subjects, for whom it was shown that IES score accounted for the reporting of psychiatric symptoms following adversity. It appeared to be the dimension of cognitive intrusion that mediated this effect, with cognitive avoidance occupying a subsidiary, reactive role. There was also evidence of specific relationships between intrusion and anxiety and somatic subscales of the GHQ, and between avoidance and depression. Despite small numbers and simple measures, the findings supported the post-disaster studies, and raises further questions about the emotional processing of adversity.

This meta-analysis evaluated the impact of cognitive behavior therapy (CBT) on the psychological adjustment of adults with spinal cord injury (SCI). A comprehensive search of six electronic databases identified 10 studies (424 participants) that met the inclusion criteria. Pre- and post-data for participants who received CBT were pooled and analyzed. Large and significant group differences were noted for measures of assertiveness, coping, self-efficacy, depression and quality of life. These data suggest that CBT has a significant positive impact on short-term psychological outcomes following SCI. Adaptation to climate change is a critical issue to many developing economies. The issue is particularly important to agriculture, a sector relying substantially on climate-sensitive resources. However, understanding of adaptation is limited in Southeast Asian contexts, including Vietnam. This thesis, therefore, investigates the attitude and behaviour of rice farmers in the Mekong Delta, a major agricultural region of Vietnam, in response to climate change. The thesis is guided by an integrated conceptual framework that was predominantly developed from protection motivation theory. The framework incorporates socio-economic and psychological factors to explain farmers’ adaptation intentions and behaviours to climate change. Focus group discussions and agricultural officer interviews generated insights into the research context and supplemented the questionnaire design. A structured questionnaire was used to interview 600 randomly chosen rice farmers in the three selected provinces in the Mekong Delta. Those provinces were identified as highly, moderately, and mildly vulnerable to climate change. The focus group discussions and agricultural officer interviews indicated that farmers were aware of climate change. However, they had limited knowledge of the importance of adaptation to their livelihoods. Barriers to farmers’ adaptation were not exclusively limited to economic factors and resource constraints. The author validated Berry’s model of acculturation (Berry, 1990a, 1990b, 1991; Berry, Kim, Power, Young, [amp] Bujaki, 1989) and examined the relation between acculturation attitudes and sociocultural and psychological adaptation among Croatian and Polish immigrants to Italy, 2 groups whose cultures are not very different from the Italian culture. Moreover, the author investigated the relation between the need for cognitive closure (NCC; Webster [amp] Kruglanski, 1994) and psychological and sociocultural adaptation. The participants completed a questionnaire including measures of sociocultural adaptation, psychological adaptation, social relationships, acculturation attitudes, and NCC. The results of a multivariate analysis of variance revealed main effects of acculturation strategies for both forms of adaptation and a main effect of NCC for psychological adaptation. The Croatian and Polish immigrants differed in the level of sociocultural adaptation but not in the level of psychological adaptation.

The study examines the relationship between immigrants’ adaptation, acculturation strategies and self-monitoring. One-hundred-and-sixty-two Polish immigrants (mean age=31.3 years, SD=7.28) living in Rome participated in the study. The majority of the participants (66.0%) were females. A Questionnaire containing scales for assessing Sociocultural adaptation, Psychological adaptation, Attitudes towards social relationships with Italians and Poles, and Self-monitoring was administered. Using adaptation indices as criteria in moderated multiple regression analyses, we found main effects of self-monitoring and of assimilation and integration strategies, and interactive effects of self-monitoring and assimilation or integration strategy. Self-monitoring was positively related both to sociocultural and psychological adaptation in all the regressions. Assimilation and integration strategies in most of cases were also positively related to both types of adaptation. Such main effects, however, were qualified by the interactive effects. As far as sociocultural adaptation is concerned, simple slope analysis showed that: (a) the positive effect of choosing assimilation is much stronger for high self-monitor immigrants than for low self-monitor ones; (b) the effect of choosing integration is positive for low self-monitor immigrants...

This paper discusses the criteria that should be considered when selecting psychological assessment tests in a research context. Traditionally attention has focused ??? and still does ??? on the stages that must govern any process of test construction/ adaptation. This work is guided by internationally accepted standards, whose scientific importance is agreed by the scientific community. However, beyond any construction/ adaptation process, the use of tests is a constant feature of psychological research, so it is of vital importance to select the tests adequately. For this reason, in this theoretical study we provide a summary of the criteria that should guide test construction/adaptation as well as some general guidelines to consider when selecting tests for psychological research. The information presented is organized into six sections, each of which corresponds to a different stage in the process of test creation: a) conceptual definition of the construct to assess; b) information about item construction and qualitative assessment; c) results of the statistical analysis of the items; d) empirical evidence of the internal structure of the test; e) results of the reliability estimation; and f) external evidence of score validity. OBJECTIVE: To determine whether an individualized counseling intervention delivered by telephone-telecounseling-feasibly improves the emotional adjustment of adults with a newly acquired spinal cord injury (SCI). DESIGN: Randomized controlled trial. SETTING: Spinal injuries unit of a rehabilitation center. PARTICIPANTS: Adults (N=40) aged 18 or older, who were recently discharged home from inpatient spinal rehabilitation, were randomly assigned to a telecounseling treatment or standard-care control group. All participants had recently received psychological treatment as inpatients in order to help assist them in adjusting to their disability. Referral to the inpatient psychology service was therefore a key indicator of participants' baseline distress levels and, consequently, their need for counseling support postdischarge. INTERVENTION: Seven telecounseling sessions were delivered over a 12-week period by a single psychologist (D.D.). Pre- and postintervention data, plus a 3-month follow-up assessment, were compared with that of an SCI control group who received standard care. MAIN OUTCOME MEASURES: Psychosocial outcome was measured using the following: Depression Anxiety Stress Scale-21; Mini International Neuropsychiatric Interview; Spinal Cord Lesion Emotional Wellbeing and Coping Strategies Questionnaires; and the Multidimensional Measure of Social Support. PURPOSE: To identify and compare perceived supportive and nonsupportive behaviors exhibited by family members and friends toward adolescents with cystic fibrosis (CF), and to examine the relationships between supportive and nonsupportive behaviors and adolescents' psychological adjustment. METHOD: Participants were 35 adolescents with CF attending the Women's and Children's Hospital in South Australia. Perceived supportive and nonsupportive behaviors were assessed using an adapted version of the Chronic Disease Support Interview. The psychological adjustment of the adolescents was assessed using the Youth Self Report Form. Repeated-measures analyses of variance were performed to compare the support provided by family members and friends. Multiple regression analyses assessed the contribution of supportive and nonsupportive behaviors for the prediction of psychological adjustment. RESULTS: Family members provided more tangible support than friends who, conversely, provided more companionship support. Overall, family members scored higher than friends on ratings for supportive behaviors. No differences were observed between family members and friends on ratings for nonsupportive behaviors. Rating of nonsupportive behaviors for family members was found to be the strongest predictor of psychological adjustment. CONCLUSIONS: Family members and friends provide different types of support. New Fatherhood and psychological distress: a longitudinal study of Australian men

Despite growing interest and concern about men's mental health during the perinatal period, we still do not know whether men are more vulnerable to mental health problems during this time. The current study is one of the first to use longitudinal, population-based data to investigate whether becoming an expectant and/or new father is associated with increases in psychological distress. We analyzed 10 waves of data collected annually (from 2001 to 2010) from the nationally representative Household, Income, and Labour Dynamics in Australia (HILDA) Survey. Over this time, 349 men were identified as new fathers (first child aged <1 year), and 224 of these men had been identified as "expectant fathers" during the previous wave. A total of 1,658 men remained "never fathers." Psychological distress was measured using the 5-item Mental Health Inventory before the partner's pregnancy, during the partner's pregnancy, and during the first year of fatherhood. Longitudinal mixed models showed no significant increase in psychological distress as a function of expectant or new fatherhood; instead, some improvement in mental health was observed. The finding suggests that expectant and new fathers are not at greater risk of poor mental health. Based on International Test Commission’s Guidelines (2010) for test translation and adaptation, we propose the administration of psychological tests to older adults in an interview format arguing that it is better suited for such population. We provide and discuss practical examples and common problems of data collection regarding: the need of adapting the translation to make cultural and psychological sense; the lack of familiarity of most elders with the psychological assessment setting and process; and, ethical concerns for this specific population.

Burden, Coping, Physical Symptoms and Psychological Morbidity in Caregivers of Functionally Dependent Family Members

OBJECTIVES: this study assessed burden, coping, physical symptoms and psychological morbidity in caregivers of functionally dependent family members. METHODS: fifty family caregivers completed self-reported measures of burden, physical symptoms, psychological morbidity and coping strategies. RESULTS: there was a significant negative correlation between coping strategies and the different clinical variables, as well as a significant positive correlation between coping strategies and duration of care. It appears that the stronger bond between caregiver and family member leads to a poorer use of adaptive coping strategies. It also appears that the deterioration of the relationship between them and the lower perceived self-efficacy are more prominent in caregivers of family members with cognitive impairment, indicating that caregivers with family members without cognitive impairment face fewer difficulties. CONCLUSION: these results emphasize the need for interventions to include coping strategies, since they are important in reducing caregivers' burden, psychological morbidity and physical symptoms.

A adaptação de instrumentos psicológicos é um processo complexo que requer elevado rigor metodológico. Por não haver consenso na literatura sobre suas etapas, o presente artigo discute alguns aspectos essenciais concernentes à adaptação transcultural de instrumentos psicológicos e propõe diretrizes aos pesquisadores sobre os diferentes passos desse processo. São apresentadas, também, algumas considerações referentes à validação do instrumento adaptado. Nesta etapa, são discutidos os aspectos referentes à estrutura fatorial do instrumento, a qual requer avaliação por meio de procedimentos estatísticos, como análises fatoriais exploratórias e confirmatórias, sendo fornecidas algumas diretrizes gerais para a validação de instrumentos psicológicos em diferentes culturas.; La adaptación de instrumentos psicológicos es un proceso complejo que requiere bastante rigor metodológico.
