Numa segunda fase...

Over 30% of primary prostate cancers contain a consensus deletion of an approximately 800 kb locus on chromosome 6q15.1. The MAP3K7 gene, which encodes TGF-β Activated Kinase-1 (Tak1), is a putative prostate tumor suppressor gene within this region whose precise function remains obscure. In this study, we investigated the role of Tak1 in human and murine prostate cancers. In 50 well-characterized human cancer specimens, we found that Tak1 expression was progressively lost with increasing Gleason grade, both within each cancer and across all cancers. In murine prostate stem cells and Tak1-deficient prostatic epithelial cells, Tak1 loss increased proliferation, migration, and invasion. When prostate stem cells attenuated for Tak1 were engrafted with fetal urogenital mesenchyme, the histopathology of the grafts reflected the natural history of prostate cancer leading from prostatic intraepithelial neoplasia to invasive carcinoma. In the grafts containing Tak1-suppressed prostate stem cells, p38 and JNK activity was attenuated and proliferation was increased. Together, our findings functionally validate the proposed tumor suppressor role of Tak1 in prostate cancer.

Membranous nephropathy (MN) is a common cause of nephrotic syndrome in older adults. The association of MN with neoplasia has been controversial, but several recent studies have shown increase incidence of cancer in patients with MN [1]. We report a case of a 49-year-old male with severe nephrotic syndrome and concomitant jejunal gastrointestinal stromal tumour (GIST). The combination of preoperative Imatinib mesylate chemotherapy and tumour excision was followed by complete resolution of proteinuria within 19 months, without specific treatment for MN. An association between MN and GIST has never previously been reported in the literature.

•Sclerosing mesenteritis, and associated inflammatory conditions of the retroperitoneum, may mimic malignancy or infection.•Attempted surgical excision of sclerosing mesenteritis and other retroperitoneal conditions often lead to a morbid and unsuccessful surgery.•These conditions are immune-mediated, and respond remarkably well to immunosuppression.

Since the end of the Cold War, Civil-Military Cooperation (CIMIC) units have become more frequently involved in peace support operations (PSOs). Moreover, they have become more deeply engaged with peoples of different national and organizational cultures. These multicultural peacekeeping environments are rife with communication difficulties. Coordination and cooperation between numerous actors present in the field such as International Organizations, Non-Governmental Organizations, media, local authorities, tribal leaders and other individuals, has proved to be very challenging. Moreover, considering the nature of contemporary international interventions, any CIMIC staff is required to adapt to a wide range of new, non-military skills which would constitute a necessary "toolbox." Culturally sensitive communications such as negotiation and media interactions are vital to effective coordination and cooperation in modern peacekeeping. Thus, CIMIC officers must know how to deal with the media; how, despite enormous cultural diversity, to successfully conduct their jobs; how to work efficiently with interpreters; and finally, how to resolve local conflicts using negotiations. Besides defining these new phenomena influencing CIMIC activities in PSOs...

This thesis is intended to contribute to the Turkish armed forces' development of a national doctrine for civil military cooperation (CIMIC). CIMIC doctrine is an increasingly significant component of peacekeeping, peace enforcement, and even combat operations. Since the end of the cold war era, the number of conflicts rose steadily, and internal conflicts became more salient to international peace and security. In addition, winning in the Clausewitzian sense--simply obtaining territory and/or political, economic, and social concessions--is now of less interest than winning "hearts and minds." Thus, during a multilateral peacekeeping operation, whether UN or NATO, the lack of an overarching strategy of civil-military cooperation (CIMIC) will undermine the overall effectiveness of the mission. Turkey is in a geo-political position to make a significant contribution to the stabilization of conflicts in its region, and recently developed a series of initiatives aimed at increasing its effectiveness in peacekeeping operations. And in May 2005 the Turkish armed forces began to develop a civil-military cooperation doctrine. This thesis is intended to make a contribution to that doctrine. The thesis compares NATO doctrine with British, Canadian...

We present the first description of amiodarone toxicity in the liver without phospholipidosis or steatosis. In doing so, we will review the various effects of amiodarone toxicity in various organs. The patient is a young adult who had cardiac reconstruction as a child for transposition of the great vessels. A needle biopsy was taken due to elevated liver enzymes. Her ALT was 188 U/L (5–50) and AST 162 U/L (5–50). Alkaline phosphatase, total bilirubin, protein, and albumin were within normal limits. A serologic panel for viral hepatitis was negative. Antinuclear antibodies were positive at 260; however, anti-smooth muscle antibody and anti-mitochondrial antibody were negative. A protein electrophoresis showed a slightly elevated beta globulin 2 level of 0.5. Quantitative immunoglobulin levels were within normal limits except for a slightly elevated IgA 409 mg/dL (60–350). Liver ultrasound was unremarkable. The clinical differential was broad and included hepatic congestion along with autoimmune hepatitis. Sections showed only ballooned hepatocytes with Mallory-Denk bodies and perisinusoidal fibrosis. Arrival to the diagnosis was possible only after careful review of the patient's medications. After discontinuation of amiodarone...
