A fase de fechamento de uma mina, se não for bem administrada, causa impactos ambientais que atingem de forma direta a população do entorno. Historicamente, as minas exauridas eram abandonadas sem que se fossem tomadas providências para reduzir riscos e impactos sócio-ambientais. Na atualidade, em diversos países, as empresas de mineração são responsáveis pela reabilitação das áreas mineradas e por tomar providências visando mitigar os impactos negativos do fechamento de minas. No entanto, muitas vezes ainda se vê uma atitude negligente por parte de certos empreendedores, abandonando a área minerada ou implantando somente medidas insatisfatórias para um adequado fechamento da mina, sob alegação de que não possuem meios para financiarem as medidas necessárias numa fase em que não há mais geração de receita, pois não se fez a provisão de recursos durante a operação do empreendimento. Tal situação é inaceitável diante dos novos rumos tomados pela humanidade, que exige, ainda que de forma incipiente, a promoção do desenvolvimento sustentável. O Diabetes Mellitus tipo 2, apresenta-se atualmente como um sério problema de saúde pública, devido as suas causas, complicações e grande incidência na população, afetando vários aspectos da vida das pessoas, provocando rupturas com modo de vida anterior e necessitando de modificações nos hábitos diários, sendo responsável por vários graus de incapacidade, o que leva a uma modificação no estilo de vida e conseqüentemente elevação dos custos com o tratamento, pois as reinternações tornam-se freqüentes, trazendo ônus para o Sistema de Saúde, para o paciente e sua família. Esta pesquisa objetiva investigar a influência do estilo de vida na reinternação hospitalar de mulheres com diabetes tipo 2. A população estudada foi composta por 50 mulheres, com faixa etária entre 45 e 65 anos, as quais foram divididas em dois grupos: Grupo Teste (composto por mulheres com duas ou mais hospitalizações entre janeiro de 2002 a dezembro de 2004) e Grupo Controle (composto por mulheres com apenas uma internação no período de estudo citado acima). Para a escolha da amostra levou-se em consideração o critério de inclusão, o qual estabelece que as mulheres dos dois grupos são assistidas pelo Ambulatório de Endocrinologia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. Contudo, a “voz” do cliente pode ser uma “voz” de insatisfação, que se manifesta através do processo de reclamação, e é sobre esta “voz” que incide este trabalho. O processo de reclamação é um processo que, pela sua complexidade, pode originar a perda de clientes e um aumento dos custos de uma empresa. Com base neste paradigma, o presente projecto consiste numa análise às reclamações efectuadas nas lojas Modelo e Continente Hipermercados, da Sonae Distribuição, de forma a procurar melhorar o seu nível e tratamento e, se possível, lançar propostas de melhoria. Para fundamentar e simplificar este estudo, recorreu-se a algumas ferramentas da qualidade, particularmente, as sete ferramentas básicas da qualidade. Continuing growth in the number of impaired elderly persons necessitates a continued reliance on nursing homes to care for at least those who are most impaired or most lacking in other supports, despite dissatisfaction over the quality of nursing home services and anxiety about the costs. Nursing home care now costs more than $30 billion annually, half of which comes from governmental sources. The Medicaid program, in particular, is central to all aspects of the nursing home industry. Private long-term care insurance is unlikely to solve the problem of nursing home financing. Rationalizing public expenditures will hinge critically on greater clarity as to just what roles nursing homes are expected to fulfill in the system of care, especially how they are supposed to relate to other services provided to Medicare beneficiaries.

Continuous voice recognition dictation systems for radiology reporting provide a viable alternative to conventional transcription services with the promise of shorter report turnaround times and increased cost savings. While these benefits may be realized in academic institutions, it is unclear how voice recognition dictation impacts the private practice radiologist who is now faced with the additional task of transcription. In this article, we compare conventional transcription services with a commercially available voice recognition system with the following results: 1) Reports dictated with voice recognition took 50% longer to dictate despite being 24% shorter than those conventionally transcribed, 2) There were 5.1 errors per case, and 90% of all voice recognition dictations contained errors prior to report signoff while 10% of transcribed reports contained errors. 3). After signoff, 35% of VR reports still had errors. Additionally, cost savings using voice recognition systems in non-academic settings may not be realized. Based on average radiologist and transcription salaries, the additional time spent dictating with voice recognition costs an additional $6.10 per case or $76,250.00 yearly. The Triple Aim—enhancing patient experience, improving population health, and reducing costs—is widely accepted as a compass to optimize health system performance. Yet physicians and other members of the health care workforce report widespread burnout and dissatisfaction. Burnout is associated with lower patient satisfaction, reduced health outcomes, and it may increase costs. Burnout thus imperils the Triple Aim. This article recommends that the Triple Aim be expanded to a Quadruple Aim, adding the goal of improving the work life of health care providers, including clinicians and staff.

This study involved two academically-gifted samples of 11th and 12th grade youth at the socioeconomic status (SES) extremes; one from an exclusive private, affluent school, and the other from a magnet school with low-income students. Negative and positive adjustment outcomes were examined in relation to multiple dimensions of perfectionism including perceived parental pressures to be perfect, personal perfectionistic self-presentation, and envy of peers. The low-income students showed some areas of relative vulnerability, but when large group differences were found, it was the affluent youth who were at a disadvantage, with substantially higher substance use and peer envy. Affluent girls seemed particularly vulnerable, with pronounced elevations in perfectionistic tendencies, peer envy, as well as body dissatisfaction. Examination of risk and protective processes showed that relationships with mothers were associated with students’ distress as well as positive adjustment. Additionally, findings showed links between (a) envy of peers and multiple outcomes (among high SES girls in particular), (b) dimensions of perfectionism in relation to internalizing symptoms, and (c) high extrinsic versus intrinsic values in relation to externalizing symptoms.

Devido ao crescente índice de chamadas para os proprietários de veículos com propósito de substituir peças com defeitos de fabricação, conhecidos com “recalls”, gerando custos e insatisfação, este trabalho visa demonstrar a importância da aplicação de ferramentas da qualidade como, “Poka-Yoke” e Análise dos Modos de Falhas e seus Efeitos (FMEA), na redução dos custos. O FMEA, é uma importante ferramenta utilizada para planejamento avançado da qualidade, indicando as possibilidades de falhas que possam vir a ocorrer nas linhas fabris durante a produção seriada, deste modo cataloga-se todos os modos de falhas passíveis de ocorrência e então os avalia um a um, atribuindo pontuações para priorizar a tratativa das respectivas ações para a mitigação ou solução, já o “Poka-Yoke” é uma ferramenta que torna possível a detecção, alerta e prevenção de erros no processo produtivo. Assim, será apresentado como a combinação destas ferramentas pode contribuir na resolução de erro na operação de identificação em processo de expedição na exportação de produtos para empresas automobilísticas e reduzir significativamente os riscos do processo. L’invalidité attribuable à la douleur représente un problème important en raison de ses coûts personnels, financiers et sociétaux. L’effort scientifique mène à l’identification des facteurs de risque pour l’évolution de la douleur vers un état qui mine la capacité de la personne affligée à vaquer à ses occupations fondamentales. Cet effort met en relief le rôle déterminant que jouent les facteurs psychosociaux à chaque stade de l’évolution vers l’invalidité en raison de la douleur. Parmi les facteurs mis en cause, se trouvent les difficultés psychologiques (dépression, anxiété, somatisation, trouble de la personnalité, catastrophisme et évitement de l’activité), l’insatisfaction au travail et le contexte de réclamation. This thesis places boundary conditions on the withdrawal model in the frontline setting of service organizations by considering continuance commitment and supervisory support as moderators of the relationship between job dissatisfaction and customer-oriented citizenship behaviors (COCBs). Departing from traditional research in the areas of the service-profit chain and employee withdrawal, the author advances our understanding of conditions that may lead frontline service employees who are dissatisfied to deposit COCBs into the organizational system. Specifically, based on principles derived from social exchange theory, high continuance commitment and high supervisory support are expected to lead to COCBs, because under this condition the benefits of performing such behaviors are increased (i.e., promotion-based, reciprocity-based), while the costs are decreased (i.e., opportunity costs). Utilizing a sample of 127 frontline employees from both the financial services and travel agency industries, the hypothesized relationships are empirically supported using moderated hierarchical regression analysis. This paper is intended to provide an assessment of the impact of the silent revolution of the last three decades on moving governments closer to people to establish fair, accountable, incorruptible and responsive governance. To accomplish this, a unique data set is constructed for 182 countries by compiling data from a wide variety of sources to examine success toward decentralized decision making across the globe. An important feature of this data set is that, for comparative purposes, it measures government decision making at the local level rather than at the sub-national levels used in the existing literature. The data are used to rank countries on political, fiscal and administrative dimensions of decentralization and localization. These sub-indexes are aggregated and adjusted for heterogeneity to develop an overall ranking of countries on the closeness of their government to the people. The resulting rankings provide a useful explanation of the Arab Spring and other recent political movements and waves of dissatisfaction with governance around the world.

Performance of electric utilities is essential for the reform of the electricity sector in the Sub-Saharan Africa (SSA) countries. The World Bank is leading a number of initiatives to improve electric utility performance in SSA. The Africa Energy unit of the World Bank recently introduced the Electric Utility Capacity Assistance Program (EUCAP) for Africa. Among other activities, the EUCAP aims to collate relevant utility performance data within the context of the Bank operations, with the dual objective of assessing the impact of reforms and motivating utility management to achieve better outcomes. In order to reach this objective, performance benchmarking approach was developed. In this approach, benchmarking starts with the selection of the Key Performance Indices (KPIs) reflecting major elements of utility day-to-day operations, including technical, operational and financial. Then a framework to compare performance of the sector in various countries and among utilities using these indices is applied.

This paper begins with a review of the broad motivations behind the New Public Sector Management (NPSM), including intrinsic differences between public and private organizations that appear to impact on incentives and performance. The experience in selected OECD countries is reviewed where the financing and delivery of health and other social services is heavily socialized with a strong public sector role, taxpayers have expressed dissatisfaction with traditional modes of public sector management, and NPSM reforms have been hotly debated. Part II of the paper then describes the NPSM paradigm in terms of three building blocks that influence the performance of public agencies and the behaviors of employees who work for them. It explains how leverage points within the NPSM paradigm are expected to create incentives for improved performance. It is when all three building blocks of the NPSM paradigm work together that synergies are expected to take place, and that continuous improvements in the performance of public agencies are expected to be generated over time. Part III illustrates five organizational strategies that can be used to introduce NPSM into public agencies in the national health system. Much of Part III refers to developing country applications.

Proceedings Paper (for Acquisition Research Program); There is increasing dissatisfaction with cost growth in major defense acquisition programs. Cost growth crowds out other investments, stresses budgets or causes schedule slips, all of which result in a military force that is less capable than previously expected. Several recent studies have recommended two categories of reforms: capital budgeting reforms seek stability in acquisition accounts, and rational cost model reforms seek to reduce the percentage increase of final cost over budget estimates. In both categories, undesirable secondary effects may be worse than the desirable primary effects; specifically, reforms that reduce cost growth may do so by driving total costs higher. This study examines these reforms and discusses their secondary effects. The paper concludes that the current practice of generating low estimates, coupled with dissatisfaction with cost growth may best serve to limit total cost.

Ugandan decentralization efforts of the 1990s represented an unusually authentic and powerful local government reform, compared to similar efforts pursued in many other low-income countries. However, over time the changing interests of the central agencies, dissatisfaction with service outcomes, and the overall dynamics of the country's governance resulted in the adoption of a number of re-centralizing policies. The objective of this report is to take stock of the fiscal and institutional arrangements for service delivery by local governments in the context of district proliferation and in view of recent trends in national public finance, as well as to identify policy options that could facilitate improved service delivery. The report finds that, while district proliferation has not had any major effect on public finances so far, it may have serious adverse effects in the future if the institutional structures and funding mechanisms of district governments are not adjusted to the new realities. In with the performance of recently privatized electricity distribution companies. Governments complain that tariffs have increased without visible improvements in service. Investors contend that they have not earned reasonable returns on their investments. Both sides often express dissatisfaction with the new independent regulatory commissions established at the time of privatization. In particular, investors argue that the commissions have not lived up to their commitments and almost always side with consumer interests. Some investors claim that the design of the new regulatory system in many developing and transition economies is fundamentally flawed. They often recommend that independent regulatory commissions be supplemented or replaced by more explicit "regulation by contract." Acompanhando as mudanças, também o mundo do trabalho se adapta, quer a nível conceptual quer a nível da forma da sua execução. Surge o trabalho em call center, com analogias ao trabalho taylorizado tanto pelas características das funções realizadas, como pelo impacto negativo que frequentemente tem junto dos trabalhadores. Nos call centres é recorrente o turnover, ou seja, a entrada e saída de trabalhadores, e este pode ser encarado como um problema para a organização se considerarmos os custos do despedimento, da contratação e da formação de novos trabalhadores. A saída do trabalhador pode ser antecedida de insatisfação com as características das funções e com aspetos organizacionais que originam cansaço e exaustão, que também podem contribuir para a insatisfação. Producing shale oil on a large scale is one of the possible alternatives for reducing dependence of the United States on imported petroleum. Industry is not producing shale oil on a commercial scale now because costs are too high even though industry dissatisfaction is most frequently expressed about "non-economic" barriers: innumerable permits, changing environmental regulations, lease limitations, water rights conflicts, legal challenges, and so on. The overall purpose of this study is to estimate whether improved technology might significantly reduce unit costs for production of shale oil in a planned large-scale industry as contrasted to the case usually contemplated: a small industry evolving slowly on a project-by-project basis. In this preliminary phase of the study, we collected published data on the costs of present shale oil technology and adjusted them to common conditions; these data were assembled to help identify the best targets for cost reduction through improved large-scale technology They show that the total cost of producing upgraded shale oil (i.e. shale oil accpetable as a feed to a petroleum refinery) by surface retorting ranges from about $18 to $28/barrel in late '78 dollars with a 20% chance that the costs would be lower than and 20% higher than that range. A cost comparison study was made of Excerpta Medica's abstracting journals, based upon actual costs to a library. Unit costs were determined for six sections of EM as compared with six corresponding abstract journals. On average, EM sections were found to be 138% more costly than corresponding abstract journals. The effects of splitting of EM journal titles were also analyzed. This practice increases the price of a total subscription to EM and makes comprehensive information retrieval more difficult. A survey of medical school librarians as users of EM points to dissatisfaction with its increasing price, particularly when it results from title splitting.
