Objective: Eating Disorder Not Otherwise Specified (EDNOS) constitute the most common eating disorder among those seeking treatment at eating disorder facilities; they are even more common among persons with eating disorders the community. This study compares the impact of applying the revised diagnostic criteria proposed by the DSM-5 workgroup, and the broad categories for the diagnosis of eating disorders (BCD-ED) proposed by Walsh and Sysko on the prevalence of EDNOS. Method: In two nationwide epidemiological studies the prevalence of eating disorders among female high school (n 5 2,028) and university students (n 5 1,020) was examined using DSM-IV criteria. We used a two-stage design, administering a questionnaire in the first stage and an interview in the second stage.Results: In the combined samples 118 cases of eating disorders (DSM-IV) were detected, of which 86 were diagnosed as EDNOS (72.9%). Application of the DSM-5 criteria reduced the number of EDNOS cases to 60 (50.8%) or to 52 (44%), when using a BMI 18.5 as cutoff for ‘‘signifi- cantly low weight’’ criterion in AN; with the use of BCD-ED criteria, only 5 (4.2%) cases of EDNOS remained. The seacliffs evolution is an important aspect to be taken in account in the evolution of the world coastline. The seacliffs can suffer erosion induced by the storm wave incidence or subaerial erosion leading to the retreat of the coastline. However the amount of sediments that come from the cliff retreat represent an important sediment source to the coastal system. In some cases it is essential to include this volume in the sediment budget balance of the studied coastal area. Many methods have been developed to monitor the evolution of seacliffs, most of them are supported by field measurements. In these work you propose the application of a new stereo photogrammetric method to reconstruct the cliff topography producing digital surface model (DSM) revealing the spatial distribution of the elevation errors. The model results are complemented by the acquisition of field data (GCP-ground control points) obtained using the DGPS (Differential Global Positioning System). This method also allows the generation of a coarse Digital elevation model (DEM) of the bottom of the seacliffs. Neste trabalho discutimos a ideia de que apesar dos manuais de diagnóstico e estatística das perturbações mentais (DSM) da Associação Psiquiátrica Americana (APA) se assumirem como categoriais, na prática, acabam por não o ser totalmente, apresentando diversas características compatíveis, sim, com uma lógica dimensional. Resumimos também aqui em treze pontos as diversas dificuldades do DSM que comprometem de certo modo a sua utilização clínica. Como pano de fundo deste trabalho está a ideia defendida por diversos psicopatologistas de que uma abordagem dimensional resolve muitos dos problemas inerentes aos sistemas de classificação de tipo categorial, em geral, e ao DSM, em particular, e de que é muito mais consentânea com a realidade clínica.

Estudos de prevalência de Transtornos de Aprendizagem (TA) em crianças e adolescentes com TDAH apresentam resultados não conclusivos, apesar da frequente comorbidade entre as síndromes. Esse artigo descreve e analisa a prevalência de TA em uma amostra referida de 270 crianças e adolescentes com TDAH atendidas no Programa de Transtornos de Déficit de Atenção/Hiperatividade/Hospital de Clínicas de Porto Alegre - Universidade Federal do Rio Grande do Sul (ProDAH/HCPA-UFRGS), de acordo com os critérios diagnósticos do Manual Diagnóstico e Estatístico de Transtornos Mentais IV - versão revisada (DSM-IV-TR). Verificou-se que 46,7% dos estudantes apresentam, pelo menos, um TA, sendo o Transtorno da Expressão Escrita o mais frequente (32,6%). Discutiu-se o potencial impacto das mudanças nos critérios diagnósticos para os TA, propostas pelo DSM-5 na prevalência desses transtornos. Embora seja possível afirmar que ocorrerão alterações nas taxas de prevalência de TA, o impacto do DSM-5 sobre a prevalência, especificidade e comorbidade do TA permanece incerto, já que há mudanças que ampliarão a abrangência dos critérios de inclusão e outras que a reduzirão.

Este artigo se propõe a analisar duas críticas referidas à publicação do DSM-V, que concentraram a atenção da comunidade internacional: a crítica realizada por Thomas Insel, diretor do National Institute of Mental Health (NIMH), e a realizada por Allen Frances, que fora o chefe do grupo de tarefas da equipe que elaborou o DSM-IV. As duas críticas serão analisadas - a partir da leitura dos cursos ministrados por Michel Foucault no Collège de France, particularmente - Os Anormais, O poder psiquiátrico e Segurança, território e população. Além destas duas críticas, será analisado também o texto de Ian Hacking, Lost in the Forest, publicado após a edição do DSM-V. Hacking propõe nesse texto não nos determos numa ou outra nova categoria diagnóstica (as árvores), mas na própria estratégia de diagnóstico apresentada no Manual como um todo (a floresta).

This study used receiver operating characteristic (ROC) methodology and discriminative analyses to examine the correspondence of the Child Behavior Checklist (CBCL) rationally-derived DSM-oriented scales and empirically-derived syndrome scales with clinical diagnoses in a clinic-referred sample of children and adolescents (N = 476). Although results demonstrated that the CBCL Anxiety, Affective, Attention Deficit/Hyperactivity, Oppositional and Conduct Problems DSM-oriented scales corresponded significantly with related clinical diagnoses derived from parent-based structured interviews, these DSM-oriented scales did not evidence significantly greater correspondence with clinical diagnoses than the syndrome scales in all cases but one. The DSM-oriented Anxiety Problems scale was the only scale that evidenced significantly greater correspondence with diagnoses above its syndrome scale counterpart —the Anxious/Depressed scale. The recently developed and rationally-derived DSM-oriented scales thus generally do not add incremental clinical utility above that already afforded by the syndrome scales with respect to corresponding with diagnoses. In this paper we review the Diagnostic and Statistical Manual of mental health (DSM), its scientific bases and utility. The concepts of “normality,” “pathology,” and boundaries between them are critically reviewed. We further use the concepts of mindfulness and mindlessness, and evidence from cognitive and social sciences to investigate the DSM clinical and social impact and we argue against its assigned overpower. We recommend including alternative perspectives to the DSM, such as mindfulness and positive psychology. We also argue for including mindfulness training in psychiatric residency and clinical psychology programs.

Esta disserta??o apresenta os algoritmos considerados estado-da-arte para gerenciamento din?mico de espectro (DSM). As t?cnicas de otimiza??o utilizadas nos algoritmos DSM s?o abordadas e brevemente discutidas para melhor entendimento, descri??o e compara??o dos algoritmos. A an?lise comparativa entre os algoritmos foi realizada considerando o ganho em taxa (kbps) obtido em simula??es. Para tanto, foi realizado em laborat?rio um conjunto de medi??es de fun??o de transfer?ncia direta e de acoplamento, posteriormente utilizadas nas simula??es dos algoritmos IWF, ISB e SCALE. Os resultados obtidos nas simula??es atrav?s do uso das fun??es de transfer?ncia medidas mostraram melhor desempenho quando comparados aos demais resultados ao considerar fun??es de transfer?ncia obtidas a partir do padr?o 1% pior caso, resultado este reflexo da aproxima??o 1% em que os pares apresentam maiores n?veis de crosstalk em todas as frequ?ncias da fun??o de transfer?ncia. Dentre os algoritmos comparados, o ISB e SCALE obtiveram desempenho semelhante em canais padronizados, ficando o IWF com o desempenho pr?ximo ao SSM. OBJECTIVE: Two randomized controlled clinical trials have shown thatLactobacillus (L) reuteri DSM 17938 reduces the duration of diarrhea in children hospitalized due to acute infectious diarrhea. This was the first trial evaluating the efficacy of L. reuteri DSM 17938 in outpatient children with acute infectious diarrhea.METHODS: This was a multicenter, randomized, single-blinded, case control clinical trial in children with acute watery diarrhea. A total of 64 children who presented at outpatient clinics were enrolled. The probiotic group received 1 × 108 CFU L. reuteri DSM 17938 for five days in addition to oral rehydration solution (ORS) and the second group was treated with ORS only. The primary endpoint was the duration of diarrhea (in hours). The secondary endpoint was the number of children with diarrhea at each day of the five days of intervention. Adverse events were also recorded.RESULTS: The mean duration of diarrhea was significantly reduced in the L. reuteri group compared to the control group (approximately 15 h, 60.4 ± 24.5 h [95% CI: 51.0-69.7 h] vs. 74.3 ± 15.3 h [95% CI: 68.7-79.9 h], p < 0.05). The percentage of children with diarrhea was lower in the L. reuteri group (13/29; 44.8%) after 48 h than the control group (27/31; 87%; RR: 0.51; 95% CI: 0.34-0.79...

In 1993, Thailand initiated a demand-side management (DSM) program to help curb electricity demand growth and promote more energy-efficient equipment and cost-effective energy services within the country. Since then, the Electricity Generating Authority of Thailand (EGAT), the national generation and transmission utility's DSM Office has developed a strong portfolio of DSM measures, including 19 DSM programs targeting a wide range of sub-sectors and end-uses, and substantially surpassed its original peak reduction and energy conservation targets. EGAT has created substantial public awareness of energy conservation and actively promoted private sector participation in promoting such services. And, EGAT's DSM Office has been recognized internationally for its success in designing DSM programs that fit within an Asian context as well as its innovation and partnershis with other agencies. Le domaine de recherche scientifique sur les cyberdépendances présente de multiples définitions de cette pathologie. À partir du matériel empirique issu des débats relatifs à la proposition d'inclure l'addiction à Internet dans le Manuel Diagnostique et Statistique des Troubles mentaux (DSM-V), ce mémoire vise à comprendre les processus sociaux bornant le développement de cette pathologie au regard de la théorie de la biomédicalisation. À cette fin, un premier travail sociohistorique retrace les voies de la biologisation progressive des pratiques sociales de communication en ligne amorcées dès 1980. Un second travail d'analyse de discours systématise ensuite le processus normatif se dégageant des controverses scientifiques liées à son inclusion dans le DSM-V. La recherche menée suggère une interprétation théorique de cet objet située à l'intersection de tendances sociales propres à la société d'information. Le projet d'inclusion révèle la volonté d'une régulation sociale effectuée à partir de la transformation technoscientifique du vivant selon des processus santéistes et néolibéraux. Notre stage d’analyse en criminologie s’est déroulé au sein du Programme des adolescents de l’Institut Philippe-Pinel de Montréal (IPPM). Notre échantillon est composé de 106 adolescents présentant un trouble des conduites (TDC) en association, ou non, à un trouble déficitaire de l’attention avec hyperactivité (TDAH) ou une maladie affective bipolaire MAB). De plus en plus de recherches s’entendent pour dire que la MAB se présenterait d’une façon atypique chez une population juvénile, ce qui explique qu’elle soit rarement identifiée chez les jeunes et qu’elle soit souvent prise pour un TDAH sévère ou un TDC avec comportements agressifs chez les délinquants irritables, impulsifs et agressifs. Certains cliniciens vont jusqu’à évoquer la possibilité que ces délinquants puissent présenter un trouble des conduites dysphorique, c’est-à-dire un type de TDC associé à une MAB typique (adulte) ou atypique (juvénile). À travers la réalisation d’une étude rétrospective de huit ans, nous avons visé à évaluer la prévalence possible de la MAB juvénile en comparant les caractéristiques des jeunes présentant un TDC avec une MAB diagnostiquée à partir du DSM-IV à celles des jeunes présentant un TDC avec une MAB juvénile potentielle identifiée à partir du CBCL. The development of the fifth edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders—the DSM-5—has reenergised and driven further forward critical discourse about the place and role of diagnosis in mental health. The DSM-5 has attracted considerable criticism, not least about its role in processes of medicalisation. This paper suggests the need for a sociology of psychiatric critique. Sociological analysis can help map fields of contention, and cast fresh light on the assumptions and nuances of debate around the DSM-5; it underscores the importance of diagnosis to the governance of social and clinical life, as well as the wider discourses critical commentaries connect with and are activated by. More normatively, a sociology of critique can indicate which interests and values are structuring the dialogues being articulated, and just how diverse clinical opinion regarding the DSM can actually be. This has implications for the considerations of health services and policy decision-makers who might look to such debates for guidance.

Background: The school-age versions of the ASEBA (Achenbach System of Empirically Based Assessment) incorporate the DSM-Oriented scales. These scales make it possible to quantify and normalize problems defined in the DSM. The objective was to study the incremental validity of the DSM-Oriented scales of the ASEBA inventories, the Child Behavior Checklist - CBCL, completed by parents of children aged 6-18 years, and the Youth Self-Report -YSR, a self-report for children/adolescents aged 11-18, over: (a) scores on the Syndromes Scales for making DSM-IV diagnoses; and (b) diagnoses obtained with structured interviews for the assessment of functioning. Method: A clinical sample of 420 children and adolescents (8-17 years) was assessed with the CBCL, and 108 adolescents were assessed with the CBCL and YSR questionnaires. All underwent a diagnostic interview, and interviewers completed a measure of global functional impairment. Results: The DSM-Oriented scales showed significant incremental validity in conjunction with the Empirical Syndrome scales for discriminating DSM-IV diagnoses, and considerable incremental validity in conjunction with the diagnoses obtained through the diagnostic interview for predicting the level of functional impairment. Background: With the imminent publication of the new edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), there has been a growing interest in the study of the boundaries across the three bulimic spectrum syndromes [bulimia nervosa-purging type (BN-P), bulimia nervosa-non purging type (BN-NP) and binge eating disorder (BED)]. Therefore, the aims of this study were to determine differences in treatment response and dropout rates following Cognitive Behavioural Therapy (CBT) across the three bulimic-spectrum syndromes. Method: The sample comprised of 454 females (87 BED, 327 BN-P and 40 BN-NP) diagnosed according to DSM-IV-TR criteria who were treated with 22 weekly outpatient sessions of group CBT therapy. Patients were assessed before and after treatment using a food and binging/purging diary and some clinical questionnaires in the field of ED. “Full remission” was defined as total absence of binging and purging (laxatives and/or vomiting) behaviors and psychological improvement for at least 4 (consecutive). Results: Full remission rate was found to be significantly higher in BED (69.5%) than in both BN-P (p < 0.005) and BN-NP (p < 0.001), which presented no significant differences between them (30.9% and 35.5%). The forthcoming DSM edition (DSM 5) will be published in May, 2013. Drafts that have been made available up to the present have launched several controversies. One of them is a possible diagnosis inflation that would generate an epidemic of false positive cases. The case of the proposed «Attenuated Psychotic Syndrome» is an important example, but others like the «Mild Neurocognitive Disorder» or the «Binge Eating Disorder» add to the list of controversial new proposals. Moreover, some methodological aspects of the whole procedure, including the imposition of a nondisclosure agreement, the lack of transparency and possible conflicts of interests have also been condemned. Historical developments of the DSM’s evolution are reviewed: DSM-III and its «diagnostic reliability revolution», DSM-IV’s validity problems and the doubts raised by DSM-V’s expected «dimensional paradigm» shift. The aim of this study is to discuss the importance assumed by the medicalization of life nowadays. After a brief survey about violence approach in main aspects of psychopathology, we underline the consequences of the operation of classification systems, promoted especially by the DSM in two aspects: degradation of the  diagnostic for a formal lifting of the symptoms and the historical accentuation on the correlation between psychopathy and crime. It is understood that violence is intrinsic and structural to human beings. The field of psychiatry, historically at odds with psychology and psychoanalysis (as the form evaluation and therapeutic), continues to sustain a classificatory (taxonomic) quadrate practice, based on characteristics and diagnostic criteria of disturbances or verified disorders, mostly empirically. The use of statistical tools in the human sciences is questionable, and what is intended in this article is to point the guise of quantitative criteria in qualitative criteria and, by extension, the common discursive practice of confusing descriptions with appreciations, the latter with evaluative and normative judgments. A circle is closed: the frequencies (statistics) define the normalities (axiological) and these are sustained in frequencies. Background and Objectives: According to existing data the term dementia was invented in the first century BC. It was introduced in the European literature in the 17th and 18th centuries AC. At the end of the 17th century, the French Encyclopedia points at ethiological implications which would later shape legal concepts. In the 19th century the Centroeuropean research develops specific nosologies until, in the 20th century, senile dementia is gradually discredited. Methods: Slightly over ten years ago, the Mild Cognitive Disorder (MCD) conceptualization was introduced as an early stage of Alzheimer's disease (AD), but the lack of coherence in relation to lesions sparked a still ongoing controversy, as the author of the MCD concept belongs to the Writing Board of DSM 5. Results: The DSM IV focuses on a categorical approach in spite of the difficulty in differentiating “normal” from “pathological” impairment at certain ages. On the other hand, the DSM 5 adopts the Dimensional System with a Mild or Severe Neurocognitive Disorder definition, which is necessarily arbitrary and imposes a statistical criterion. Un tema de gran interés a lo largo de la historia de la psiquiatría, ha sido el relacionado a los sistemas diagnósticos. El Manual Diagnóstico y Estadístico de los Trastornos Mentales (DSM por sus siglas en inglés) y la Clasificación Internacional de Enfermedades (CIE), integran los criterios diagnósticos comúnmente utilizados en la práctica psiquiátrica. El DSM en su cuarta edición revisada, es ya insuficiente para el trabajo clínico actual, por lo que se ha estado trabajando en la configuración de lo que será el DSM-V (quinta edición); no obstante, aún y con toda la planeación, se teme que no se logren cubrir todas las necesidades de la psiquiatría moderna. La primera edición del DSM fue publicada en la década de 1950; esta versión del manual, y la subsiguiente, reflejaban la psiquiatría psicodinámica de esa época. En la década de 1980 se publicó la tercera edición y su versión revisada, en las que predominaban las consideraciones descriptivas y sindromáticas. Años después fue publicada la cuarta versión y una revisión posterior de este manual en el año 2000. Los primeros esfuerzos para iniciar el proceso de integración del DSM-V se iniciaron desde 1999. Desde ese año, hasta el 2007, se planificó la investigación y la documentación técnica por medio de un programa de investigación. Del año 2004 al 2007 se llevó a cabo la segunda fase del proyecto denominada «perfeccionamiento del programa de investigación para el DSM-V». De enero a mayo del 2010 se efectuó una prueba piloto con diferentes poblaciones y las revisiones propuestas por los grupos de trabajo. En el 2011 se revisarán los criterios diagnósticos propuestos y las medidas dimensionales. En el año 2012 se preparará el proyecto de texto final...
