A pesquisa investiga as concepções e modo de atuação das profissionais envolvidas no atendimento em estimulação precoce, com bebês de zero a um ano completo de idade e verifica como elas visualizam a interação mãe-filho no referido atendimento. Foram entrevistadas vinte e duas profissionais (sete professoras, seis fonoaudiólogas, cinco fisioterapeutas e quatro psicólogas), em seis escolas de ensino especial, da cidade de Curitiba, Paraná. Após análise qualitativa dos dados, observa que as profissionais: a) seguem um modelo de atuação com enfoque tecnicista, focalizado na criança e em sua deficiência, com ênfase em atividades e exercícios terapêuticos; b) desconhecem a facilitação da interação mãe-filho como importante na promoção do desenvolvimento infantil, relegando a presença da mãe no atendimento a um segundo plano; c) sentem falta de mais formação na área em que atuam e reconhecem que não estão preparadas para lidar com a família. Conclui propondo uma mudança do enfoque atual do atendimento em estimulação precoce, por outro focalizado na criança e na mãe, a fim de facilitar a interação entre ambos e dessa forma promover o desenvolvimento infantil. Esta tese de doutorado é um estudo de caso sobre uma família com um bebê com deficiência visual e a efetividade de uma intervenção precoce, dividindo-se em quatro artigos. O primeiro objetivou revisar a produção científica sobre intervenção precoce na área da deficiência visual. O segundo artigo investigou o duplo impacto da prematuridade e da deficiência visual, examinando as reações emocionais dos pais de um bebê nesse contexto e seu impacto na parentalidade. O terceiro artigo investigou a efetividade de uma intervenção precoce com foco na sensibilidade materna – e paterna, analisando o pré e o pós-intervenção. O último estudo trata-se de um relato de experiência que examina o processo da intervenção. Early intervention (EI) has been characterized by considerable advances in its domain, which has had great repercussions in the implementation of the family-centered approach. These changes have had implications in the practices and in the adoption and learning of new values that should be implemented in EI. This study evaluates the professional perspectives regarding familycentered practices in EI programs in Portugal. The results highlight the importance of effective collaboration and coordination between health, education, and social services and the importance of providing child and family support in a natural context. Reseach about the impact of the Early Intervention System en Alentejo, Portugal. The investigation project “Evaluation of early intervention impact in Alentejo” was the consequence of several actions that, in the past 7 years, provided the three districts of Alentejo with an early intervention child development net, that was pioneered in Portugal and that brings together the initiatives from the regional services of the Health, Education and Social Security Ministries along with private non-profitable institutions of social care. For the first time, it was possible to create a large, uniform net, covering a large area, with a coordinated structure and articulated work and intervention models, laying down as the main purpose support to children with developmental disturbances or in risk, and the families of those children.

OBJECTIVES: The objective of this article is to discuss the rationale/background for early intervention in bipolar disorder. METHOD: Narrative review. RESULTS: There are often significant delays before the diagnosis of bipolar disorder is made and effective management initiated. Growing evidence from both preclinical and clinical literature points to a clear need for improved early identification and early intervention in bipolar disorder. Increasing efforts are being applied to the identification of those at high risk of onset of bipolar disorder. It is hoped that identification of an early prodrome of illness will allow preventative measures to be taken. CONCLUSIONS: There is a clear rationale for improved early identification and early intervention in bipolar disorder.

The rise of the early intervention paradigm in psychotic disorders represents a maturing of the therapeutic approach in psychiatry, as it embraces practical preventive strategies which are firmly established in mainstream health care. Early intervention means better access and systematic early delivery of existing and incremental improvements in knowledge rather than necessarily requiring dramatic and elusive breakthroughs. A clinical staging model has proven useful and may have wider utility in psychiatry. The earliest clinical stages of psychotic disorder are non-specific and multidimensional and overlap phenotypically with the initial stages of other disorders. This implies that treatment should proceed in a stepwise fashion depending upon safety, response and progression. Withholding treatment until severe and less reversible symptomatic and functional impairment have become entrenched represents a failure of care. While early intervention in psychosis has developed strongly in recent years, many countries have made no progress at all, and others have achieved only sparse coverage. Current thinking about intervention increasingly accepts the premise that early childhood experience crucially determines health and wellbeing and the attainment of competences at later ages, and that investment in the early years will be reflected in improved education, employment, and even national productivity. While this logic in itself appears unassailable, and is for many outcomes backed by strong research evidence, decisions about just what deserves public investment are complex. For indigenous societies, sources of early risk are situated in family contexts whose potentials for resilience are often not well understood and where languages for intervention have not been created. This paper outlines aspects of an early intervention project in progress on the Tiwi Islands to highlight themes in indigenous parenting and to examine the prospects for developing culturally competent interventions which can help alleviate some sources of early childhood risk. It illustrates the families and the problems they face and considers how intervention can be approached through engagement of community members within strategies for development of new services. It is vital that children with a disability have their condition recognised early and subsequently receive access to high quality early intervention (EI) services. Research shows that EI results in significant benefits and sustainable outcomes for these children. This report seeks to examine how the Federal government can best serve the needs of children with disabilities and their families through the provision and funding of EI services. This report focuses on an expansion of the Helping Children with Autism (HCWA) package as a possible Commonwealth government funding model, to include children who are deaf or hearing impaired; blind or visually impaired; or who have Cerebral Palsy (CP) or Down Syndrome (DS). Families of children with disabilities are at greater risk of a number of social and psychological stresses, including mental and physical health problems, marital problems and divorce, poor housing and increased expenses coupled with reduced income. Additionally, it is estimated that bringing up a disabled child cost three times that of bringing up a non-disabled child. The purpose of this research was to determine whether initial developmental delay, site of intervention, frequency of intervention, age of the child, socio-economic status (SES), gender and ethnicity significantly predict developmental gains in a group of children enrolled in an early intervention setting. The records of 134 children enrolled in an inner-city program in Miami, Florida were reviewed for inclusion in this study. ^ Demographic variables, site placement and treatment frequencies were collected during a retrospective chart review. Level of delay was expressed using the developmental quotient and developmental gain was calculated using the mean gain on age equivalent scores or developmental tests. A multiple regression analysis was performed to determine which of the above variables significantly predicted developmental gains. Multivariate analysis compared developmental gains for all the developmental domains based on intervention site (center versus home-based) while controlling for developmental delay. ^ Children made greater developmental gains if they had higher developmental quotients and if they were younger at the time services were initiated. Frequency of intervention significantly improved developmental outcomes in children attending center-based programs. Children attending center-based programs also made significantly greater gains in gross motor skills compared to children attending home-based programs. ^ These findings emphasize the importance of early screening and referral of children with developmental delay and adjusting intervention for the child's developmental quotient. Children should receive intense treatment to maximize results. Decisions regarding program placement should be individualized according to the child's unique developmental pattern. Policy and program decisions affecting the curriculum of a child in early intervention need to reflect these multivariate considerations. ^

However, successfully fulfilling this role is a challenge, and this is especially in deprived urban areas. Aims: To inform a complex intervention to support GPs in this important role, we aim to identify the key areas in which general practice can help address youth mental health and strategies to enhance implementation. Methods: We conducted a modified Delphi study which involved establishing an expert panel involving key stakeholders / service providers at two deprived urban areas. Early intervention is a multidisciplinary practice which seeks to respond both to the needs of children with developmental disabilities and those who are at risk, surpassing the old models of early stimulation. The problem of deciding which children and families should be supported, and the requirements for planning of interventions, stress the importance of a clear characterization, or diagnosis, of children and contexts. In the model underlying the network of Early Intervention, locally based, implemented in a wide region of Portugal, are emphasized relational and contextual dimensions of development and risk. The authors are developing practices and tools for better eligibility of cases and technical guidance intervention focused more on the actual needs of the child, family and context. Here are presented the results of the use of DOEI - Diagnosis Organization in Early Intervention - in the characterization of the development problems and risk of a total of 1.169 children and their contexts.

OBJECTIVE: This article aims to review Latin America's early intervention services in psychosis and to shed light into their challenges and particularities. METHOD: An internet-based search comprising medical societies' websites, published articles, and major universities' websites was conducted and the results were critically discussed. RESULTS: Latin American countries are profoundly deficient in specialized early intervention services. Our search found seven target services, four of which are based in urban areas of Brazil, inside tertiary hospitals or universities. Among the initiatives advanced by these centers, there are partnerships with the public educational system and other community-based efforts toward knowledge transfer. On the other hand, several challenges remain to be overcome, especially in relation to their expansion, which is necessary to match the existing demand.

Utilizamos neste estudo uma metodologia de cariz qualitativo – Estudo de Caso. O nosso grupo de participantes foi constituído por quatro famílias acompanhadas há mais de seis meses por uma equipa de Intervenção Precoce a operar no distrito de Braga. It is currently under an indefinite embargo pending publication by Springer.; Purpose: The objective of this review is to report on recent developments in youth mental health incorporating all levels of severity of mental disorders encouraged by progress in the field of early intervention in psychotic disorders, research in deficiencies in the current system and social advocacy. Methods: The authors have briefly reviewed the relevant current state of knowledge, challenges and the service and research response across four countries (Australia, Ireland, the U.K. and Canada) currently active in the youth mental health field. Results: Here we present information on response to principal challenges associated with improving youth mental services in each country. Australia has developed a model comprised of a distinct front line youth mental health service (Headspace) to be implemented across the country and initially stimulated by success in early intervention in psychosis; in Ireland, Headstrong has been driven primarily through advocacy and philanthropy resulting in front line services (Jigsaw) which are being implemented across different jurisdictions; in the UK a limited regional response has addressed mostly problems with transition from child-adolescent to adult mental health services; and in Canada a national multi-site research initiative involving transformation of youth mental health services has been launched with public and philanthropic funding...

CONTEXTO:A detecção e a intervenção precoces na esquizofrenia são uma área nova de pesquisa em grande expansão. OBJETIVO: Revisar a literatura atual sobre a detecção e intervenção precoces em esquizofrenia. MÉTODO: A literatura foi pesquisada por meio do PubMed, manuais e referências bibliográficas utilizando termos como: "alto risco", "intervenção precoce", "detecção precoce", "esquizofrenia", "psicose precoce". RESULTADOS: Utilizando os paradigmas de prevenção seletiva e de prevenção indicada de outras áreas da Medicina, a detecção e a intervenção precoces na esquizofrenia procuram estudar as manifestações iniciais da esquizofrenia, antes que o quadro psicótico tenha se manifestado claramente. O reconhecimento da importância dos primeiros tempos de vida no desenvolvimento da criança mudou significativamente os campos da educação, saúde e proteção social. A Intervenção Precoce representa a preocupação intencional de intervir ao nível do desenvolvimento, nomeadamente junto das crianças mais pequenas que se encontram em risco de ter o seu percurso afetado. The prenatal period and the early years of life have an extraordinary importance on the physical and psychological well-being not only in the infancy, but throughout the life cycle. There is strong empirical evidence that early life is highly vulnerable to the negative effects of adverse experiences or toxic stress as maternal prenatal anxiety or child maltreatment. Research in the field of developmental neurobiology provides important keys about the mechanisms across these experiences affect the process of child development provoking alterations and dysfunctions in brain architecture. Such alterations tend to be persistent and increase the risk of physical, cognitive, social and emotional problems along infancy, adolescence and adulthood. The evidences clearly support the need and social relevancy of implementing early intervention preventive programs for children and families who are at risk for experiencing toxic stress. Such policies and programs should begin as early as possible in order to reduce or avoid the need of most costly and less effective remediation programs.

The provision of early intervention services for vulnerable children and their families is now both accepted and expected by the international community. This article considers the importance of a developmental perspective as an essential guide to early intervention service systems. Emphasized in this framework are three critical features: relationship formation, the continuity of interventions, and the comprehensiveness of interventions. Guidance to early intervention systems design with respect to structural and values principles is also discussed. Future advances in early intervention may well depend upon the merging of these perspectives to create policy initiatives to enhance early intervention systems.
