This study assesses the implementation of a protocol to prevent pressure ulcers in ICU inpatients. This prospective, descriptive and exploratory study verifies the incidence of pressure ulcers following the implementation of a prevention protocol. Data were collected from April 17th to July 15th 2009. The incidence observed in this study (23.1%) was below that reported in a similar study developed in the same institution (41.02%) before the implementation of the protocols to assess risk and prevent pressure ulcers. The prevention protocols are essential tools that have an impact on controlling the incidence of pressure ulcers, when used consistently.

O processo acelerado de modernização científica e tecnológica tem gerado novas formas de conhecimento, o que tem levado instituições a rever o modo de formação dos profissionais. Dentre as várias características necessárias para este novo profissional, destacam-se a capacidade de buscar e utilizar novas informações, saber comunicar-se (nacional e internacionalmente), usar os recursos tecnológicos para a melhoria de vida, desenvolver a crítica e desempenhar a profissão de forma contextualizada e em equipe. Podemos afirmar então que o cuidado de enfermagem precisa ter uma abrangência maior. E assim, ao considerar a formação do enfermeiro, é que buscamos conhecer a situação atual do ensino, no que se refere à prevenção e tratamento de úlceras por pressão, nos cursos de graduação em enfermagem de oito Instituições Públicas de Ensino Superior do estado de São Paulo. Objetivo: validar as intervenções da classificação de enfermagem NIC para o diagnóstico Risco de Integridade da Pele Prejudicada, em pacientes em risco de úlcera por pressão (UP). Método: a amostra foi composta por 16 enfermeiras experts. Os dados foram coletados em instrumento contendo a caracterização das participantes, além de uma tabela com as intervenções e a definição de cada uma delas, bem como uma escala Likert que foi pontuada pelas experts. Os dados foram analisados estatisticamente, utilizando-se cálculo de média ponderada (MP). O estudo foi aprovado em Comitê de Ética e Pesquisa (56/2010). Resultados: validaram-se nove intervenções como prioritárias (MP ≥0,80), dentre elas prevenção de UP (MP=0,92); 22 como sugeridas (MP>0,50 e <0,80) e 20 foram descartadas (MP≤0,50). Purpose: To estimate the frequency of use of pressure-redistributing support surfaces (PRSS) among hip fracture patients and to determine whether higher pressure ulcer risk is associated with greater PRSS use. Design and Methods: Patients (n = 658) aged ≥65 years who had surgery for hip fracture were examined by research nurses at baseline and on alternating days for 21 days. Information on PRSS use and pressure ulcer risk factors was recorded at each assessment visit. Other information was obtained by interview and chart review. Results: A PRSS was observed at 36.4% of the 5,940 study visits. The odds of PRSS use were lower in the rehabilitation setting (adjusted odds ratio [OR] 0.4, 95% confidence interval [CI] 0.3–0.6), in the nursing home (adjusted OR 0.2, 95% CI 0.1–0.3), and during readmission to the acute setting (adjusted OR 0.6, 95% CI 0.4–0.9) than in the initial acute setting. There was wide variation in frequency of PRSS use by admission hospital, even after adjusting for pressure ulcer risk factors. The relationships between PRSS use and pressure ulcer risk factors were not strong. Implications: In this study of hip fracture patients, adherence to guidelines for PRSS use was low and was based more on facility-related factors than on patient risk. There is an urgent need for health care providers to improve strategies for the prevention of pressure ulcers in high-risk patients.

The problem facing the incidence of pressure ulcers (UP) in the hospital environment especially in the intensive care unit (ICU), although it is an old and frequent event in our professional practice, it is not notified in the researches as much as it should be. We observed a tendency to invest in therapeutical and in studies about the production of sophisticated new bandages. Few, however, are the investments in research on preventive measures in order to prevent or at least slow down the development of lesions. In this sense, the study aimed to analyze the correlation between nursing care and the risk of developing UP measured by the Braden scale in ICU patients. This is a descriptive study of longitudinal quantitative approach. The project obtained a favourable opinion from the Ethics Committee of HUOL (no 486/10). Data collection was carried out in the Hospital of Unimed in Natal during six months in 2011. The sample was of 32 patients hospitalized in ICU for over four days. The results were processed in SPSS 15.0 for descriptive statistics and inferential statistics. Purpose: Pressure ulcers are a high cost, high volume issue for health and medical care providers, having a detrimental effect on patients and relatives. Pressure ulcer prevention is widely covered in the literature, but little has been published regarding the risk to patients in the radiographical setting. This review of the current literature is to identify findings relevant to radiographical context. Methods: Literature searching was performed using Science Direct and Medline databases. The search was limited to articles published in the last ten years to remain current and excluded studies containing participants less than 17 years of age. In total 14 studies were acquired; three were excluded as they were not relevant. The remaining 11 studies were compared and reviewed. Discussion: Eight of the studies used ‘healthy’ participants and three used symptomatic participants. Nine studies explored interface pressure with a range of pressure mat technologies, two studies measured shear (MRI finite element modelling, and a non-invasive instrument), and one looked at blood flow and haemoglobin oxygenation. CONTEXT AND OBJECTIVE: The prevalence of pressure ulcers varies according to geographic region and population group, such as the institutionalized elderly. The aim of this study was to identify the prevalence of pressure ulcers among elderly people living in long-stay institutions. DESIGN AND SETTING: Cross-sectional study in six long-stay institutions for the elderly in São Paulo, Brazil. METHODS: Demographic and clinical data were collected in six long-stay institutions on two visits to each institution between May and August 2007, during which all elderly patients with pressure ulcers were evaluated. The Braden scale was used to identify the risk of developing pressure ulcers and the National Pressure Ulcer Advisory Panel (NPUAP) stages for classifying the pressure ulcers. Statistical analysis was performed using the chi-square test, Student's t-test and Fisher's exact test. RESULTS: There was no significant difference in the results between visits. The population was 181 elderly people in May and 184 in August: 23 had pressure ulcers in May (prevalence of 12.7%) and 17 in August (prevalence of 9.2%). OBJECTIVE: to validate the Nursing Intervention Classifications (NIC) for the diagnosis 'Risk of Impaired Skin Integrity' in patients at risk of pressure ulcers (PU). METHOD: the sample comprised 16 expert nurses. The data was collected with an instrument about the interventions and their definitions were scored on a Likert scale by the experts. The data was analyzed statistically, using the calculation of weighted averages (WA). The study was approved by the Research Ethics Committee (56/2010). RESULTS: nine interventions were validated as 'priority' (WA ≥0.80), among them Prevention of PU (MP=0.92); 22 as 'suggested' (WA >0.50 and <0.80) and 20 were discarded (WA ≤0.50). CONCLUSIONS: the prevention of PU results from the implementation of specific interventions related to the risk factors for development of the lesion, with implications for nursing practice, teaching and research.

Pressure ulcers are still considered a serious problem, especially in elderly people and in situations of chronic-degenerative diseases. The purpose of this study was to identify the literature production about actions of prevention and treatment performed by nurses published from 1999 to 2004, describing the knowledge produced about the theme. This is a descriptive literature review of nursing journals indexed in LILACS and MEDLINE about the theme from 1999 to 2004. Data collection occurred from May to June, 2005. Afterwards, the material was read exhaustively, which yielded three aspects to be studied: prevention of pressure ulcers, treatment of pressure ulcers and nursing care of pressure ulcers. Es un estudio de caso realizado en un hospital de enseñanza, en el periodo de diciembre de 2012 a febrero de 2013, con objetivo de identificar el conocimiento de los profesionales de enfermería acerca de las úlceras por presión en dos unidades quirúrgicas, con destaque para aspectos referentes a la prevención. Participaron 25 profesionales de enfermería y se utilizó un instrumento validado que considera conocimiento adecuado a los que obtuvieron 90% de aciertos o más. La media de aciertos de las unidades fue igual a 73,88%. Los errores más comunes: el concepto de fase III de las úlceras por presión y la indicación de masaje en regiones de proeminencia ósea, elevación de la cama a un ángulo superior a 30º, uso de guantes d’água, almohadas redondas, reposicionamiento de pacientes sentados. Los resultados apuntan la necesidad de actualización sobre el tema úlceras por presión de acuerdo con los guidelines.; A case study was undertaken in a teaching hospital, in December 2012 – February 2013, aiming to identify the knowledge of the nursing professionals regarding pressure ulcers in two surgical units, with emphasis on aspects relating to prevention. This study aimed to identify nursing team’s knowledge on treatment and preventive measures of pressure ulcers at a hospital in the Minas Gerais State, Brazil. Fifteen nursing technicians and assistants from medical and surgical clinics participated in this study. The following preventive measures were identified in their answers: changes in the patients’ lying position (100%); moisturizing the skin with oil (60%); eggshell mattress (80%); cushion (40%); massage (20%), among others. As for treatments, coverings, medicines and solutions were quoted. As for the competence of these professionals, 33,3% stated that they followed medical prescription, and none of them quoted nursing prescription. The results of this study demonstrated that the nursing team in this hospital knows several recommended practices both for prevention and treatment of PU, although some mentioned old fashioned and misled behaviors. Results: It was found that the occurrence of PU are common mainly in the environment of high complexity, due to decreased mobility and restrictions in bed. Conclusion: There is still a need for scientific production directed primarily at the prevention of UP with respect to move in bed, for the strengthening of Evidence-Based Practices in the field of nursing.

The Pressure Ulcer and an underestimated complication in critically ill patients bedridden, since it can lead to several complications for the patient's condition. The aim of this study and look through the PICO strategy in nursing journals the best available evidence for the customer care of high complexity to prevent pressure ulcers through changing positions. Computerized literature search included in the period from 2007 to 2012, in scientific databases PubMed and the Portal of Nursing Journals. It was concluded that the position change is one of the most effective ways to prevent pressure ulcers, and of course if done with other prevention measures increases the chances of preventing the onset of further pressure ulcer.

The ulcers are under pressure from several comorbidities and outcomes are related to the patient who is the sole dependence on care in the ICU care in preventing or reducing its ulcer disease is more intense, and requires skill of nurses, study, knowledge, needs constant search for the renewal of methods to reduce the frequency of occurrence or the severity increased. This study aimed to identify scientific production association in the period 2004 to 2008 at about the methods available for the prevention of pressure sores in ICU and how nurses can contribute to the prevention of pressure ulcers in the ICU, 10 (ten) articles were selected and analyzed and concluded that the commitment to bring the reality of the institution to the patient's need, in the hands of nurses sometimes becomes an art, based on his practice and experience and an incentive to search.

The pressure ulcers (PU) are an important problem of health. The best strategy to avoid them is prevention. There are many scales of risk assessment to prevention of pressure ulcers, that complement the examination of nurses. The present study one is about a comparison enters two methods evaluators of risk factors for developing pressure ulcers: Waterlow scale and the Braden scale in order to know which has better efficiency by focusing on providing preventive care to the hospitalized client. Aimed at analyzing articles published in scientific journals and international Online Brazilian Journal of Nursing and the electronic database LILACS and PubMed, from 2005 to 2009. The methodology used was a literature review with qualitative approach, being 11 articles selected and analyzed. It was concluded that both scales are effective in assessing risk for development of UP and the use of scales increases the intensity and effectiveness of intervention and prevention.

This study obtained through a literature review, whose main goal was to obtain data for use by nursing staff for preventive care, systematic and individualized in hospitalized patients at risk for developing pressure ulcers, identifying risk factors to this, and often these are presented in bedridden patients in an institution providing health services.
